摘要
目的分析急性脑卒中偏瘫患者不同时间点标准化量表的评估意义及预后生活能力的影响因素。方法68例首发急性脑卒中偏瘫患者。使用标准化Barthel指数评分量表(BI)、Rankin量表(RS)及Montgomery Asberg抑郁量表(MADRS)评测患者发病时及发病48 h、15 d、90 d的日常生活活动能力(ADL)、神经功能及精神心理状况。结果患者发病90 d与发病时、发病48 h、15 d的BI、RS及MADRS评分比较,差异有统计学意义(P<0.05),发病48 h与15 d时的各项评分比较,差异无统计学意义(P>0.05);发病48 h出现肺炎、尿失禁及深度感觉障碍是影响脑卒中偏瘫患者生活能力预后的危险因素。结论发病48 h出现肺炎、尿失禁和深度感觉障碍是影响急性脑卒中偏瘫患者日常生活活动能力的危险因素。不同时间点标准化量表能够有效评估急性脑卒中偏瘫患者的日常生活活动能力。
Objective To analyze the evaluation significance of standardized scale at different time points in patients with a cute stroke hemiplegia and the influencing factors of prognostic life ability.Methods We selected 68 patients with first episode acute stroke hemiplegia.Standardized Barthel index scoring scale(BI),Rankin scale(RS)and Montgomery Asberg depression scale(MADRS)were used to evaluate the activities of daily living(ADL),neurological function and mental status of patients at the onset and after 48 hours,15 days and 90 days of onset.Results There were statistically significant differences in scores of BI,RS and MADRS between patients after 90 days of onset and at the onset,after 48 hours of onset,and after 15 days of onset(P<0.05),but there was no statistically significant differences between patients after 48 hours and 15 days of onset(P<0.05).The incidence of pneu monia,urinary incontinence and deep sensory disturbance after 48 hours of onset are the risk factors affecting the life ability prognosis of stroke hemiplegia patients.Conclusion The incidence of pneumonia,urinary incontinence and deep sensory disturbance after 48 hours of onset are the risk factors affecting the daily activities of patients with hemiplegia after acute stroke.Standardized scales at different time points can effectively evaluate the daily living activities of these patients.
