摘要
目的观察慢性乙型肝炎(CHB)患者血清谷氨酸脱氢酶(GLDH)活性、载脂蛋白A-Ⅰ(ApoA Ⅰ)、载脂蛋白B100(ApoB100)含量的动态变化及临床意义。方法我院收治的CHB患者50例(CHB组),同期健康体检者50例(对照组),均予常规保肝、抗病毒等治疗,持续治疗28 d。所有患者治疗前后均行血清GLDH活性、ApoA Ⅰ、ApoB100含量检测及Child分级评分,分析血清GLDH活性、ApoA Ⅰ、ApoB100含量与Child分级评分的关系。结果与对照组相比,CHB组血清GLDH活性及Child分级评分明显较高,ApoA Ⅰ、ApoB100含量明显较低(P<005)。治疗后,CHB组血清GLDH活性及Child分级评分较治疗前明显降低,血清ApoA Ⅰ、ApoB100含量较治疗前明显升高(P<005)。治疗前后CHB组血清GLDH活性与Child分级评分呈正相关,ApoA Ⅰ、ApoB100含量与Child分级评分呈负相关(P<005)。血清GLDH活性及ApoA Ⅰ、ApoB100水平诊断CHB的曲线下面积分别为0933、0863、0722。结论 CHB患者血清GLDH活性升高,ApoA Ⅰ、ApoB100含量降低,三者的检测不仅对CHB有重要辅助诊断价值,还有助于疗效评估。
Objective To observe the dynamic changes of serum glutamate dehydrogenase(GLDH)activity,contents of apoli poprotein A I(ApoAI)and apolipoprotein B100(ApoB100)in patients with chronic hepatitis B(CHB)and to explore their clinical significance.Methods Fifty patients with CHB admitted to our hospital and 50 healthy subjects undergoing physical examination at the same time were selected as CHB and control groups,respectively.All patients were treated with routine liver protection and antiviral therapy for 28 days.Serum GLDH activity,contents of ApoA and ApoB100 as well as Child grading score before and after treatment were measured in all patients.The relationship between serum GLDH activity,contents of ApoAI and ApoB100 and Child grading score was analyzed.Results Compared with the control group,serum GLDH activity and Child grading scores in the CHB group were significantly higher while contents of ApoAI and ApoB100 were significantly lower(P<0.05).After treatment,serum GLDH activity and Child grading scores in the CHB group were significantly decreased while serum contents of ApoAI and ApoB100 were significantly increased(P<0.05).The serum GLDH activity of the CHB group was positively correlated with Child grading score before and after treatment while contents of ApoAI and ApoB100 were negatively correlated with Child grading score(P<0.05).The values of area under the ROC curve of serum GLDH activity,ApoA I and ApoB100 levels in the diagnosis of CHB were 0.933,0.863 and 0.722,respectively.Conclusion The serum GLDH activity of CHB patients is increased while the contents of ApoAI and ApoB100 are decreased.The detec tion of the three indexes not only has important diagnostic value for CHB,but also contributes to the evaluation of therapeutic efficacy.
