老年慢性支气管炎合并Ⅱ型呼吸衰竭采用有创-无创序贯机械通气治疗的效果及安全性观察 被引量:6

Efficacy and Safety of Invasive and Noninvasive Sequential Mechanical Ventilation in Senile Chronic Bronchitis with Type II Respiratory Failure

摘要 目的:探讨无创-有创序贯机械通气治疗老年慢性支气管炎合并Ⅱ型呼吸衰竭的临床疗效。方法:随机将2014年1月~2017年3月收治的74例老年慢性支气管炎合并Ⅱ型呼吸衰竭患者分为两组,A组采用有创机械通气治疗,B组采用有创-无创序贯机械通气治疗,比较两组的临床疗效。结果:B组治疗后的呼吸频率、SaO2、PaCO2、PaO2、有创通气时间、总通气时间、住院时间、再插管率、VAP率与院内死亡率明显少于A组,组间比较差异有统计学意义(P<0.05)。结论:有创-无创序贯机械通气治疗老年慢性支气管炎合并Ⅱ型呼吸衰竭,能有效改善患者的呼吸通气功能,缩短其治疗及住院时间,且治疗安全,更适合用于临床推广。 Objective: To explore the clinical efficacy of invasive and noninvasive sequential mechanical ventilation in the treatment of chronic bronchitis with type II respiratory failure in the elderly. Methods: 74 elderly patients with chronic bronchitis and type II respiratory failure admitted from January 2014 to March 2017 were randomly divided into two groups. Group A was treated with invasive mechanical ventilation. Group B was treated with invasive and noninvasive sequential mechanical ventilation. The clinical efficacy of two groups were compared. Results: The respiratory rate, SaO2, PaCO2 and PaO2 indexes, invasive ventilation time, total ventilation time, hospitalization time, reintubation rate, VAP incidence and hospital mortality in group B were significantly lower than those in A group after treatment, the difference between groups was statistically significant(P<0.05). Conclusion: Invasive and noninvasive sequential mechanical ventilation in treating senile chronic bronchitis with type II respiratory failure can effectively improve the patients’ respiratory ventilation function, shorten their treatment and hospitalization time, and the treatment is safe, more suitable for clinical promotion.
