摘要
目的探讨心脏瓣膜置换术后低心排综合征发生的危险因素,并给予针对性护理措施。方法选取2017年12月至2018年11月我院收治的287例瓣膜置换术患者为研究对象,术后并发低心排综合征87例,采用单因素与多因素logistic回归分析低心排综合征发生的危险因素。结果糖尿病史、吸烟史、术前心律失常对低心排发生的影响差异无统计学意义(P>0.05),术后心律失常、肾功能不全、心胸比、体外循环时间、呼吸机辅助时间、主动脉阻断时间、重症监护时间、失血量比、体质量指数对低心排综合征的发生影响有统计学意义(P<0.05)。logistic回归分析显示,体外循环时间>100 min、主动脉阻断时间>1 h、失血量比>20%、体质量指数<19 kg/m 2为瓣膜置换术后低心排综合征发生的独立危险因素。结论对瓣膜置换术后低心排综合征发生相关因素进行分析,明确独立危险因素,同时给予针对性护理干预措施,降低低心排综合征发生率。
Objective To explore the risk factors of low cardiac output syndrome after valve replacement and to provide targeted nursing measures.Methods We selected 287 cases of patients undergoing valve replacement in our hospital from December 2017 to November 2018 as a study object.87 cases of patients with low cardiac output syndrome were treated with univariate analysis and multivariate logistic regression to analyze the risk factors for the occurrence of the low cardiac output syndrome.Results There was no significant difference in the influence of diabetes history,smoking history,and preoperative arrhythmia from low cardiac output(P>0.05).Postoperative arrhythmia,renal insufficiency,cardiothoracic ratio,extracorporeal circulation time,ventilator assist time,and aortic occlusion time,intensive care time,blood loss ratio,body mass index had a statistically significant effect on the incidence of low cardiac output syndrome(P<0.05).Logistic regression analysis showed that extracorporeal circulation time>100 min,aortic occlusion time>1 h,blood loss ratio>20%,and body mass index<19 kg/m 2 were the independent risk factors of the occurrence of low cardiac output syndrome after valve replacement.Conclusion The factors related to the occurrence of low cardiac output syndrome after valve replacement were analyzed,and independent risk factors were identified.At the same time,targeted nursing interventions were given to reduce the incidence of low cardiac output syndrome.
