摘要
目的探究全产程分娩镇痛在足月妊娠分娩中的镇痛效果,以及对产妇和新生儿安全性的影响。方法回顾120例足月妊娠分娩产妇临床资料,按照分娩镇痛时间的不同将其分成全产程镇痛组与第一产程镇痛组,比较两组分娩过程中的疼痛程度、产程时间、分娩方式以及产妇、新生儿情况。结果全产程镇痛组产妇在第二产程屏气用力时、胎头娩出时VAS评分低于第一产程镇痛组,第二产程的时间长于第一产程镇痛组,差异均有统计学意义(P<0.05);两组产妇的分娩方式、产时发热、产后出血量以及新生儿出生后1、5 min的Apgar评分,宫内窘迫率比较差异无统计学意义(P>0.05)。结论全产程镇痛能有效缓解产妇在第二产程的疼痛程度,且并未明显改变产妇分娩方式和新生儿结局。
Objective To evaluate the pain relieving efficacy of full-course labor analgesia for delivery of full-term pregnancy and its effect on maternal and neonatal safety.Methods The clinical data of 120 full-term pregnant parturients were retrospectively reviewed.According to the different time of labor analgesia,120 full-term pregnant parturients were divided into the full-course labor analgesia group and the first-stage labor analgesia group.The degree of pain,the duration of labor,the mode of delivery,the situation of parturients and newborns in the two groups were compared.Results The VAS score of the full-course labor analgesia group was lower than that of the first-stage labor analgesia group in holding breath and fetal head delivery during the second labor,and the second labor was longer than that of the first-stage labor analgesia group.The differences were statistically significant(P<0.05).No significant differences were found between the two groups in mode of delivery,maternal fever,postpartum hemorrhage,Apgar scores at 1 and 5 min,and intrauterine distress rate after birth(P>0.05).Conclusions Full-course of labor analgesia can effectively alleviate the pain of parturients in the second stage of labor without engendering any significant changes to the mode of delivery and the outcomes of the neonates.
