摘要
目的探讨使用主动脉内球囊反搏(IABP)辅助介入治疗急性心肌梗死合并心衰的效果。方法将东莞康华医院2015年3月—2018年10月收治的70例患者随机分为研究组与对照组,每组各35例。对照组给予常规抗血小板治疗、支架介入治疗及改善心肌代谢等,研究组在对照组治疗基础上,联合使用IABP辅助行介入治疗。观察两组治疗效果,同时在治疗前、治疗后3 d检测两组患者N末端B型利尿钠肽原(NT-proBNP)、左室舒张末期内径(LVEDd)及左室射血分数(LVEF)等心功能指标变化情况。结果研究组治疗总有效率为94.28%,明显高于对照组的总有效率(P<0.05);治疗3 d后两组患者NT-proBNP均较治疗前降低,LVEDd、LVEF较治疗前升高,但是研究组降低/升高幅度明显大于对照组(P<0.05);治疗后两组患者不良反应的发生率上升无显著差异(P>0.05)。结论采用IABP辅助介入治疗急性心肌梗死伴心力衰竭的效果显著,可改善患者的心功能,值得推广。
Objective To investigate the effect of intra-aortic balloon counterpulsation(IABP)assisted interventional therapy for acute myocardial infarction complicated with heart failure.Methods 70 cases of research objects from March,2015 to October,2018 in the hospital were randomly divided into research group(n=35)and control group(n=35).The control group were given routine anti platelet therapy,interventional treatment and improved myocardial metabolism,and the observation group were treated with combina tion of intraaortic balloon counterpulsation auxiliary line of interventional therapy based on the control group.Treatment effects of two group were observed.At the same time,3d detection N-terminal B diuretic natriuretic peptide(NT-proBNP),left ventricular end di astolic diameter(LVEDd)and left ventricular ejection fraction(LVEF)and other changes in cardiac function indicators before and af ter treatment of the two groups were compared.Results The total effective rate in the study group was 94.28%,which was significantly higher than that in the control group(P<0.05).3 days after treatment,NT-proBNP in both groups was lower than before treatment,and LVEDd and LVEF were higher than before treatment,but the decrease/increase in the study group was significantly greater than that in the control group(P<0.05).After treatment,the incidence of adverse reactions in the two groups was significantly different(P>0.05).Conclusion IABP is effective in the treatment of acute myocardial infarction with heart failure,which can improve the cardi ac function of patients.So it is worth promotion.
