摘要
目的探讨2型糖尿病肾病(DN)患者血清同种异体移植物炎性因子-1(AIF-1)的变化及其临床意义。方法选取2018~2019年首都医科大学附属北京友谊医院收治的180例2型糖尿病肾病患者(观察组)进行前瞻性研究,根据尿白蛋白排泄率(UAER)情况将其分为正常白蛋白尿组(NA组,n=59)、微量白蛋白尿组(MA组,n=61)和大量白蛋白尿组(LA组,n=60);另选取同期健康体检者60例作为对照组。收集所有研究对象24 h尿进行UAER检测,并采集静脉血进行肿瘤坏死因子α(TNF-α)和AIF-1检测,比较各组间UAER、TNF-α和AIF-1水平,以及对DN患者UAER、TNF-α和AIF-1间的关系进行Pearson相关性分析。结果观察组患者UAER、TNF-α和AIF-1水平分别为(196.40±202.54)mg/24 h、(25.09±10.58)pg/ml和(225.06±121.50)pg/ml,较对照组(5.93±1.39)mg/24 h、(12.22±2.76)pg/ml和(79.61±16.91)pg/ml明显升高,差异具有统计学意义(P<0.05)。NA组、MA组和LA组患者UAER、TNF-α和AIF-1水平分别为(6.93±1.79)mg/24 h、(118.74±23.98)mg/24 h、(461.66±100.87)mg/24 h,(16.08±3.33)pg/ml、(20.44±4.09)pg/ml、(38.69±4.49)pg/ml,(102.72±36.70)pg/ml、(211.95±37.25)pg/ml、(358.69±93.55)pg/ml,除NA组UAER较对照组比较差异无统计学意义(P>0.05)外,其他指标组间比较差异均具有统计学意义(P<0.05)。Pearson相关性分析显示,DN患者AIF-1与UAER、TNF-α间的关系呈正相关性(r=0.822、0.772,P<0.01)。结论DN患者血清AIF-1水平升高,且与UAER和TNF-α呈正相关性,AIF-1极有可能成为DN诊治的新生物学标志物。
Objective To investigate the changes of serum allograft inflammatory factor-1(AIF-1)in patients with type 2 diabetic nephropathy and its clinical significance.Methods 180 patients with type 2 diabetic nephropathy(observation group)were selected for prospective study,which were divided into normal albuminuria group(NA group,n=59),microal buminuria group(MA group,n=61)and large albuminuria group(LA group,n=60)according to the condition of UAER in urine.Another 60 healthy people in the same period were selected as control group.Urine samples of all subjects were collected for UAER detection for 24 hours,and venous blood samples were collected for TNF-αand AIF-1 detection.The levels of UAER,TNF-αand AIF-1 were compared among the groups,and the relationship among UAER,TNF-αand AIF-1 in DN patients was analyzed by Pearson correlation.Results The levels of UAER,TNF-αand AIF-1 in the observation group were(196.40±202.54)mg/24 h,(25.09±10.58)pg/ml and(225.06±121.50)pg/ml respectively,which was significantly higher than that of control group(5.93±1.39)mg/24 h,(12.22±2.76)pg/ml and(79.61±16.91)pg/ml,with statistical significance(P<0.05).The levels of UAER,TNF-αand AIF-1 in NA,MA and LA groups were(6.93±1.79)mg/24 h vs.(118.74±23.98)mg/24 h vs(461.66±100.87)mg/24 h,(16.08±3.33)pg/ml vs.(20.44±4.09)pg/ml vs.(38.69±4.49)pg/ml,(102.72±36.70)pg/ml vs.(211.95±37.25).pg/ml vs.(358.69±93.55)pg/ml,where has no significant difference in UAER between NA group and control group(P>0.05),but has significant difference in other indicators between groups(P<0.05).Pearson correlation analysis showed that there was a positive correlation between AIF-1 and UAER,TNF-αin patients with DN(r=0.822,0.772,respectively;P<0.01).Conclusion The level of AIF-1 in serum of patients with DN increased,and positively correlated with UAER and TNF-α,which can be likely to become a new biomarker for diagnosis and treatment of DN.
