摘要
目的探讨不同严重程度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)老年患者睡眠结构特点与微觉醒指数变化。方法 75例老年患者经多导睡眠监测仪监测,根据呼吸暂停低通气指数(AHI)不同分为OSAHS轻度、中度、重度组及鼾症对照组,对比各组间睡眠结构与微觉醒指数的不同。结果与鼾症对照组比较,OSAHS各组N1%、MAI、呼吸相关MAI显著增加(P<0.05),N3%及REM%明显下降(P<0.05);OSAHS各组间比较,重度组N1%明显高于轻、中度组(P<0.05),N2%、N3%明显低于轻、中度组(P<0.05);随着病情严重程度的加重,MAI、呼吸相关MAI显著增加(P<0.05);睡眠结构与AHI、ESS密切相关,而与年龄、BMI无相关性。结论老年OSAHS患者存在睡眠结构紊乱与睡眠片段化,且与其病情程度有关。
Objective To investigate the changes of sleep architecture and microarousal index in elderly patients with OS-AHS.Methods A total of 75 elderly patients were monitored by PSG and divided into four groups:snore,mild,middle and severe according to AHI.The differences in sleep architecture and microarousal index between the groups were analyzed.Results Compared with the snore group,there were significantly increased in N1%,MAI and MAI with disordered breathing,significantly decreased in N3%and REM%in OSAHS groups.In comparison with three OSAHS groups,N1%in aevere group was significantly higher than that in mild and moderate groups.On the contrary,N2%and N3%in severe group were significantly lower than the other two groups.With the more severity of OSAHS,MAI and respiration-related MAI were gradually increased.Sleep architecture was closely related to AHI and ESS,but had no correlation with age and BMI.Conclusion Elderly patients with OSAHS have disordered sleep architecture and fragmented sleep,which is associated with the severe degree of the lesion as indicated by AHI.
