摘要
目的:探索痰湿证多囊卵巢综合征(PCOS)患者发生代谢综合征(MS)的最佳预测指标。方法:通过对455例PCOS患者MS各组分、4种脂蛋白比值以及LAP的ROC曲线进行分析,比较各指标对MS的预测价值。结果:痰湿证PCOS患者中MS各组分的发生率从高到低依次为:腹型肥胖、血脂异常、血压异常、空腹血糖异常。ApoB/ApoA、LDL/HDL、LAP、TG/HDL、TC/HDL与DBP、WC、BMI、FBG、2h PG、HOMA-IR、FINS、2h INS、TG、TC、LDL、ApoB呈正相关,与ApoA、HDL呈负相关(P<0.05)。TG/HDL的ROC曲线下面积(AUC)和约登指数分别为0.89、0.66,均为最大值,诊断MS的价值最高。TG/HDL的AUC与TG、TC/HDL、HDL、LDL/HDL、LAP的比较,差异有统计学意义(P<0.05)。结论:TG/HDL对痰湿证PCOS患者MS的发生预测价值最大,尤其当TG/HDL截断值≥1.53时,需及时干预,避免MS的发生,提高其生活质量。
Objective: To find out the best predictor of metabolic syndrome(MS) in phlegm-dampness syndrome of PCOS. Methods: The prediction value of MS was compared by analyzing the ROC curves of MS components, the ratio of four lipoproteins and LAP. Results: The incidence of each component of the metabolic syndrome in patients with phlegm-dampness syndrome of PCOS from high to low were as follows: abdominal obesity, dyslipidemia, abnormal blood pressure, impaired fasting glucose. ApoB/ApoA, LDL/HDL, LAP, TG/HDL, TC/HDL were positively correlated with DBP, WC, BMI, FBG, 2 h PG, HOMA-IR, FINS, 2 h INS, TG, TC, LDL and ApoB, with a negative correlation with ApoA and HDL, and the difference was statistically significant(P<0.05). The AUC and Youden index of TG/HDL were respectively 0.89 and 0.66, which was the maximum value, and the value of MS diagnosis was the highest. Compared with the AUC of TG/HDL, TG, TC/HDL, HDL, LDL/HDL and LAP five indicators were statistically different(P<0.05). Conclusion: TG/HDL had the highest predictive value for the occurrence of MS in patients with phlegm-dampness. Especially when cut-off value of TG/HDL is ≥1.53, timely intervention should be taken to avoid the occurrence of MS and improve the quality of life.
