摘要
目的探讨ICU患者万古霉素首剂给药后血药浓度变化情况,分析肌酐清除率对血药浓度的影响。方法选取自2013年10月1日至2014年8月1日入住ICU并明确诊断为革兰阳性球菌感染,应用万古霉素治疗的12例患者为研究对象。万古霉素首剂给药后监测患者血药浓度,并观察肌酐清除率对血药浓度的影响。结果药物静滴结束3.0 h(用药4.6 h)后,91.7%(11/12)的患者血药浓度降至10μg/ml以下。常规剂量给予万古霉素首剂间期,超过60%的时间血药浓度<10μg/ml。肌酐清除率对万古霉素血药浓度未见明显影响。结论常规给予ICU患者万古霉素的首剂间期,血药浓度未达到有效抗菌浓度。
Objective To investigate the changes of blood concentration of ICU patients after the first dose of vancomycin,and to analyze the effect of creatinine clearance rate on blood concentration.Methods A retrospective study was performed on 12 cases of patients with gram-positive coccal infection who were admitted in ICU from October 1st 2013 to August 1st 2014.After the first dose of vancomycin,the serum concentration was monitored and the effect of creatinine clearance rate on serum concentration was observed.Results After the end of the drip for 3.0 hours(4.6 hours),the plasma concentration of 91.7%(11/12)of the patients decreased to less than 10μg/ml.The plasma concentration of vancomycin was less than 10μg/ml when the routine dose was given over 60%of the time between the first dose.No significant effect of creatinine clearance on the plasma concentration of vancomycin was observed.Conclusion Vancomycin was routinely given to ICU patients during the first dose interval,and the blood concentration did not reach the effective antibacterial concentration.
