摘要
为了探讨趋化因子17 (CCL17)、CXC趋化因子配体12 (CXCL12)及降钙素基因相关肽(CGRP)在急性脑出血患者血清中的动态变化,本研究选择2017年1月至2018年5月期间在我院就诊的急性脑出血患者112例作为自发性脑出血(ICH)组,选择30例健康志愿者作为对照组。所有患者发病后24 h内入院,分别于入院时、入院后第3天、第7天、第14天,采用ELISA法检测患者外周血清CCL17、CXCL12、CGRP水平。应用Pearson相关性分析患者入院时CCL17、CXCL12、CGRP与患者脑出血量、病情严重程度的关系。研究结果表明,ICH组患者入院时、入院后第3天、第7天、第14天时CGRP水平均低于对照组志愿者,且CCL17和CXCL12水平高于对照组志愿者(p<0.05)。在入院后当日,ICH组患者血清CGRP水平即达到最低值,而在入院后第3天时,ICH组患者血清CCL17和CXCL12水平达到最高值,与其他检测点获得的数据相比,存在统计学差异(p<0.05)。根据多田公式计算ICH患者脑出血量,其中57例患者为少量组、29例患者为中量组,26例患者为大量组。大量组患者外周血清CGRP水平明显低于少量和中量组患者(p<0.05);同时,大量组患者外周血清CCL7和CXCL12水平高于少量和中量组患者(p<0.05)。根据患者病情严重程度,61例患者为轻度组、24例患者为中度组,27例患者为重度组。重度组患者外周血清CGRP水平明显低于轻度和中度组患者(p<0.05);重度组患者外周血清CCL17和CXCL12水平高于轻度和中度组患者(p<0.05)。经Pearson相关性分析,入院时ICH患者外周血清CGRP与CCL17和CXCL12均呈负相关性(r=-0.679,-0.382, p<0.05)。而CCL17和CXCL12呈正相关性(r=0.319, p<0.05)。本研究结论初步表明,CCL17、CXCL12、CGRP参与了急性脑出血的病理生理过程,有望成为病情严重程度的参考指标。
To investigate the dynamic changes of chemokine(CCL17), C-X-C chemokine ligand12(CXCL12), and calcitonin-gene-related peptide(CGRP) in plasma of patients with acute intracerebral hemorrhage, 112 patients with acute intracerebral hemorrhage hospitalized within 24 h in our hospital as intracerebral hemorrhage(ICH) group and30 healthy volunteers as control group were selected from January 2017 to May 2018. The blood samples were obtained at 0, 3 d, 7 d, 14 d after the onset of acute intracerebral hemorrhage. The levels of plasma CCL17,CXCL12, CGRP were detected by ELISA. The correlations between CGRP, CCL17 or CXCL12 and blood volume,disease severity were analyzed. Patients in ICH group had lower CGRP level and higher CCL17 and CXCL12 levels than patients in control group at 0, 3 d, 7 d, 14 d after the onset of acute intracerebral hemorrhage(p<0.05).The plasma CGRP level of patients in ICH group at administration and CCL17, CXCL12 levels of patients at 3 d after admission were lowest than levels on the other time points(p<0.05). According to the Coniglobus formula,there were 57 patients with a little of blood volume, 29 patients with moderate blood volume, and 26 patients with a large of blood volume. Patients with a large of blood volume had lowest CGRP level and highest CCL17 and CXCL12 levels among patients in 3 groups(p<0.05). While there were 61 patients with mild disease, 24 patients with moderate disease, and 27 patients with severe disease. Patients in severe group had lowest CGRP level and highest CCL17 and CXCL12 levels among patients in mild, moderate and severe groups(p<0.05). By Pearson analysis, the serum CGRP level was negatively related with CCL17 and CXCL12(r=-0.679,-0.382, p<0.05), and the serum CCL17 level was positively related with CXCL12(r=0.319, p<0.05). It was suggested that CCL17,CXCL12, CGRP be closely related with acute intracerebral hemorrhage, which could be the possibly sensitive indexes for diagnosis of the severity of patients with acute intracerebral hemorrhage.
