阴沟肠杆菌头孢菌素酶耐药基因分析 被引量:1

Analysis of Ampcase resistance gene in Enterobacter cloacae

摘要 目的通过检测本院阴沟肠杆菌临床抗生素耐药率,了解产头孢菌素酶(AmpC酶)的阴沟肠杆菌分子流行病情况。方法纸片扩散法检测本院分离的189株阴沟肠杆菌细菌耐药,选取头孢西丁耐药的菌株行经典Hodge法鉴定产Ampc酶菌株,PCR扩增阳性菌株Ampc酶基因。结果 189株阴沟肠杆菌主要来源于呼吸内科、神经内科和重症医学科等科室,药敏结果显示对头孢类抗生素具有较高的耐药率,对碳青霉烯类抗生素普遍高敏,对头孢西丁耐药率为17.46%;Hodge法确证实验表明33株可产AmpC酶,PCR扩增出DHA基因型17株,ACT-1型13株,ADC型3例,ACC型和MOX型未检出。结论阴沟肠杆菌产AmpC酶菌株现象较为严重,本院大多为DHA和ACT-1基因型。 Objective To investigate the molecular epidemiological situation of Enterobacter cloacae producing Ampcase by detecting the antibiotic resistance rate in our hospital. Methods The paper diffusion method was used to detect the resistance of 189 strains of Enterobacter vaginis bacteria isolated in this hospital. The strain of producing Ampcase was identified by the classic Hodge method with cefoxitin resistance, and the positive strain Ampc enzyme gene was amplified by PCR. Results 189 strains of Enterobacter cloacae were mainly derived from the departments of respiratory medicine, neurology, and ICU. The drug sensitivity results showed that the epispore antibiotics had a high resistance rate, and they were generally highly sensitive to carbene antibiotics. The drug resistance rate to cefoxitin was 17.46%;Hodge test showed that 33 strains produced Ampcase. 17 strains of DHA genotype, 13 strains of ACT-1, and 3 cases of ADC type were amplified by PCR, and ACC and MOX were not detected. Conclusion Ampcase strain produced by Enterobacter cloacae is more severe. Most of them are DHA and ACT-1 genotypes.
