绝经后子宫内膜癌合并感染对HE4和VEGF及炎症因子水平的影响 被引量:11

Effect of postmenopausal endometrial carcinoma complicated with infection on the levels of HE4, VEGF and inflammatory factors

摘要 目的探讨绝经后子宫内膜癌合并感染对人附睾蛋白4(Human epididymis protein 4,HE4)、血管内皮生长因子(vascular endothelial growth factor,VEGF)及炎症因子水平的影响。方法选取2013年2月-2017年12月于石家庄市第一医院妇产科行手术治疗的106例子宫内膜癌患者为病例组,选择同时期诊断为子宫腺肌瘤已绝经患者100例为对照组。取病例组宫颈分泌物,直接涂片及进行病原菌培养后,将病例组分为感染组和未感染组,检测并对比分析三组患者HE4、VEGF及炎症因子水平的差异。结果病例组的患者血清中的HE4、VEGF分别为(24.3-435.9,279.8)pm/L、(219.6±33.1)ng/ml,均高于对照组患者的(8.2-73.5,32.8)pm/L、(27.2±12.4)ng/ml,病例组中感染组的HE4、VEGF分别为(24.3-435.9,268.9)pm/L、(221.5±32.9)ng/ml,均高于非感染组患者的(28.7-387.6,178.4)pm/L、(187.7±31.4)ng/ml(P<0.05);病例组的患者血清中的炎症因子白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、白细胞介素-8(interleukin-8,IL-8)、白细胞介素-10(interleukin-10,IL-10))指标均高于对照组的患者,病例组中感染组的IL-6、TNF-α、IL-8指标高于非感染组患者(P<0.05)。结论子宫内膜癌患者合并感染能刺激机体高度表达HE4、VEGF以及炎症因子(IL-6、TNF-α、IL-8),为临床早期发现并有效治疗子宫内膜癌合并细菌感染、提高患者预后以及生存质量提供依据。 OBJECTIVE To investigate the effect of postmenopausal endometrial carcinoma complicated with infection on the levels of human epididymis protein 4(HE4),vascular endothelial growth factor(VEGF)and inflammatory factors.METHODS Totally 106 patients with endometrial cancer who had surgical treatment in our department from Feb.2013 to Dec.2017 were selected as the case group.In the same period,100 patients diagnosed as uterine adenomyosis were selected as the control group.After taking cervical secretion from the case group,followed by direct smear and culture of pathogens,the case group was divided into infected group and uninfected group.The levels of HE4,VEGF and inflammatory factors in the three groups were analyzed and compared.RESULTS The serum levels of human epididymis protein E4(HE4)and vascular endothelial growth factor(VEGF)in the case group were(24.3-435.9,279.8)pm/L and(219.6±33.1)ng/ml,both higher than those in the control group which were(8.2-73.5,32.8)pm/L and(27.2±12.4)ng/ml.The HE4 and VEGF levels in the infected group were(24.3-435.9,268.9)pm/L and(221.5±32.9)ng/ml,both higher than those in the uninfected group which were(28.7-387.6,178.4)pm/L and(187.7±31.4)ng/ml.The differences were significant(P<0.05).The serum levels of inflammatory factors such as interleukin-6(IL-6),Tumor Necrosis Factor-α(TNF-α),interleukin-8(IL-8)and interleukin-10(IL-10)in the case group were significantly higher than those in the control group.In the case group,IL-6,TNF-α,IL-8 in the infected group were significantly higher than those in the uninfected group(P<0.05).CONCLUSION Patients who have endometrial cancer combined with bacterial infection can stimulate the body to highly express HE4,VEGF and inflammatory factors(IL-6,TNF-α,IL-8).This can provide the basis for early detection and effective treatment of endometrial cancer with bacterial infection,as well as improvement of the patients’prognosis and quality of life.
