摘要
目的对比分析器械辅助复位微创髓内钉内固定术和切开复位髓内钉内固定术治疗成人股骨干骨折的疗效,并探讨其可操作性和安全性。方法将40例新鲜成人股骨干骨折患者按照入院先后顺序随机分为试验组和对照组,分别采用器械辅助复位微创髓内针内固定和切开复位髓内钉内固定治疗,对2组失血量、手术时间、骨折愈合时间、临床疗效优良率、并发症发生情况进行比较。40例患者术后均随访12个月。结果试验组与对照组比较,手术时间较短,失血量极少,骨折愈合时间较短,差异均有统计学意义(P均<0.05)。术后4周、术后12周、术后6个月膝关节功能按Lysholm膝关节功能评分评定试验组均明显优于对照组(P均<0.05)。结论与切开复位相比,器械辅助复位微创髓内钉治疗成人股骨干骨折具有出血更少、手术时间更短、骨折愈合更快、并发症更少等优点,值得临床进一步推广应用。
Objective It is to compare the efficacy of instrument-assisted reduction minimally invasive intramedullary nailing fixation and open reduction and intramedullary nailing fixation in the treatment of adult femoral shaft fractures,and to explore its operability and safety.Methods Forty adult patients with fresh femoral shaft fractures were randomly divided into two groups according to the order of admission:test group treated with instrument-assisted reduction minimally invasive intramedullary intra-needle fixation and control group treated with open reduction and intramedullary nail fixation.The blood loss,operation time,fracture healing time,clinical efficacy rate and the occurrence of complications between the two groups were compared.The forty patients were followed up for 12 months.Results Compared with the control group,the test group had less blood loss,shorter operation time and fracture healing time,the differences were significant(P<0.05).The knee function in the test group was better than that in the control group according to Lysholm knee function at 4 weeks,12 weeks and 6 months after operation.Conclusion Compared with open reduction,instrument-assisted reduction minimally invasive intramedullary nail has the advantages of less bleeding,shorter operation time,faster fracture healing and fewer complications in the treatment of adult femoral shaft fractures,which is worthy of further clinical application.
