摘要
国医大师段富津教授将清代医家王清任活血化瘀名方--身痛逐瘀汤进行加减,临床用于治疗风湿血瘀证,疗效显著。其应用本方治疗风湿血瘀证的辨证要点为肢体关节疼痛,舌质紫暗明显。临床应用时常加味药物有活血化瘀药姜黄、赤芍、延胡索等,补气药黄芪等,祛风湿药汉防己、海桐皮等,常减味药物为没药、香附、牛膝等。对其临床加减应用这些药物的经验进行了总结,并对加味药物的功用、临床常用剂量等进行了详细的论述,最后选取典型病例,附以按语,对段富津教授应用身痛逐瘀汤治疗风湿血瘀证的思路进行探讨,以期对现代临床用身痛逐瘀汤治疗风湿血瘀证有借鉴指导意义。
Professor DUAN Fu-jin,a TCM master,added and subtracted Shentong Zhuyu Decoction,a famous prescription of promoting blood circulation and removing blood stasis by WANG Qing-ren,a doctor of Qing dynasty,and used it clinically to treat rheumatic blood stasis syndrome with remarkable effect.The key points of syndrome differentiation in treating rheumatic blood stasis syndrome are limb joints pain and dark purple tongue.In clinical application,the commonly added medicines include drug for invigorating blood circulation and eliminating stasis such as Curcuma longa,radix paeoniae rubra,corydalis yanhusuo,drug for qi-tonifying such as Astragalus membranaceus,rheumatism-dispelling medicines such as Radix stephaniae tetrandrae,cortex erythrinae.The commonly reduced medicines are myrrh,rhizoma cyperi,Achyranthes bidentata,etc.The experience of clinical application of these drugs was summarized,and the function and dosage of the modified drugs were discussed in detail.Finally,typical cases were selected and the idea of professor DUAN Fu-jin’s application of Shentong Zhuyu Decoction in treating rheumatic blood stasis syndrome was discussed with a note,in order to provide a reference for modern clinical application of Shentong Zhuyu Decoction.
