摘要
目的探讨集束化护理对机械通气患者机械通气时间和呼吸机相关性肺炎发生率的影响。方法随机选取该院重症监护室(ICU)2015年4月—2017年4月收治的80例机械通气患者,依据护理方法分为集束化护理组(n=40)和常规护理组(n=40)两组,对两组患者的机械通气时间、ICU住院时间、住院费用、呼吸机相关性肺炎、院内感染发生情况及死亡情况进行统计分析。结果集束化护理组患者的机械通气时间、ICU住院时间(3.1±0.1)d、(4.0±0.6)d均显著短于常规护理组(9.0±1.2)d、(9.1±1.3)d(t=4.303,3.182,P<0.05),住院费用(6 985.3±250.1)元显著低于常规护理组(15 130.2±290.4)元(t=2.776,P<0.05),呼吸机相关性肺炎、院内感染发生率、死亡率17.5%(7/40)、22.5%(9/40)、10.0%(4/40)均显著低于常规护理组32.5%(13/40)、52.5%(21/40)、22.5%(9/40)(χ^2=7.35,9.35,5.02,P<0.05)。结论集束化护理较常规护理更能有效缩短机械通气患者的机械通气时间,降低患者的呼吸机相关性肺炎发生率。
Objective To investigate the effect of cluster nursing on the mechanical ventilation time and the incidence of ventilator-associated pneumonia in mechanical ventilation patients. Methods Eighty patients with mechanical ventilation admitted to our intensive care unit (ICU) from April 2015 to April 2017 were randomly selected. According to the nursing method, they were divided into cluster nursing group (n=40) and routine nursing group (n=40). In the two groups, the mechanical ventilation time, ICU hospitalization time, hospitalization expenses, ventilator-associated pneumonia, incidence of nosocomial infection and death were statistically analyzed. Results The mechanical ventilation time and ICU hospitalization time (3.1±0.1)d and (4.0±0.6)d were significantly shorter in the cluster nursing group than in the conventional nursing group (9.0±1.2)d, (9.1±1.3)d (t= 4.303, 3.182, P<0.05), hospitalization expenses (6 985.3±250.1)yuan were significantly lower than the conventional care group (15 130.2±290.4)yuan(t=2.776, P<0.05), ventilator-associated pneumonia, incidence of nosocomial infection, mortality rates of 17.5% (7/40), 22.5% (9/40), and 10.0% (4/40) were significantly lower than the conventional care group by 32.5% (13/40), 52.5% (21/40), and 22.5% (9/40) (χ^2=7.35, 9.35, 5.02, P<0.05). Conclusion Clustered nursing can effectively shorten the mechanical ventilation time of patients with mechanical ventilation and reduce the incidence of ventilator-associated pneumonia in patients with conventional ventilation.
