摘要
目的:了解肿瘤患者出院带抗肿瘤药物情况及风险,为促进患者安全用药提供参考。方法:由医院信息系统调取2017年12月1日至2018年5月31日肿瘤患者出院带抗肿瘤药物的医嘱信息,从患者、科室、药品、剂型与贮存、剩余量五个方面进行分析。结果:出院带抗肿瘤药物患者共计4949人,分布于34个科室;共涉及抗肿瘤药物86种,其中主要品种23种,以细胞毒类药物最多(共18种,占比78.26%),大多为注射用药;在78种品规的注射用药中,59种品规出现剩余量现象,总剩余量以依托泊苷注射液、注射用放线菌素D、亚叶酸钙注射液为著。结论:肿瘤患者存在出院带抗肿瘤药物(尤其是细胞毒类药物)继续治疗的实际需求,但在药品院外流通中存在诸多隐患,应采取措施规避潜在风险。
Objective: To explore the anticancer drugs prescribed for discharged cancer patients and its potential risks, in order to promote safety drug use. Methods: Information of cancer patients discharged from hospital with anticancer drugs from December 1, 2017 to May 31, 2018 was collected from hospital information system (HIS). Characteristics of patients, clinical departments, varieties of drugs, dosage forms and storage conditions, and drug residuals were analyzed. Results: There were totally 4949 patients discharged from 34 departments with anticancer drugs. A total of 86 kinds of anticancer drugs were involved, in which 23 kinds of drugs were on the list of main varieties. Among the 23 kinds of main anticancer drugs, cytotoxic drugs (18 kinds, accounting for 78.26%) were the most and most of them were injections. In 78 kinds of injections, 59 kinds of injections had residuals, and the top 3 kinds of drugs with residuals were etoposide injection, dactinomycin for injection and calcium folinate injection. Conclusion: There are medical demands for discharged cancer patients to take anticancer drugs (especially cytotoxic drugs) for continuous treatments. However, potential risks of transportation, storage, and out-of-hospital use of anticancer agents should be paid more attentions to and necessary measures should be taken.
