摘要
目的:评价瑞芬太尼+右美托咪定全麻应用于经皮肾镜碎石术(PCNL)麻醉中的效果及对术中出血量及麻醉期间不良反应的影响。方法:取研究时段2017年5月至2018年12月我院接收的50例PCNL的患者开展研究。对其临床资料进行回顾性分析,以不同的麻醉方式将50例患者分为对照组与试验组,每组25例。对照组采用瑞芬太尼麻醉,试验组采用瑞芬太尼联合右美托咪定,对比镇静与疼痛评分、手术观察指标、不良反应发生率。结果:镇静评分与对照组相比,试验组较高(P<0.05);疼痛评分、不良反应发生率与对照组相比,试验组较低(P<0.05)。手术时间、术后清醒时间试验组短于对照组,而术中出血量试验组少于对照组(P<0.05)。结论:PCNL采用瑞芬太尼+右美托咪定全麻,麻醉效果显著,镇痛效果较佳,不但会减少术中出血量,还会降低不良反应发生率,临床应用价值较高。
Objective:To evaluate the effect of remifentanil+dexmedetomidine general anesthesia on percutaneous nephrolithotripsy(PCNL)anesthesia and its effect on intraoperative blood loss and adverse reactions during anesthesia.Methods:50 patients with percutaneous nephrolithotripsy admitted to our hospital from May 2017 to December 2018 were selected for the study period.The clinical data were analyzed retrospectively,and 50 patients were divided into control group and experimental group with different anesthesia methods,25 cases in each group.Remifentanil anesthesia was used in the control group,and remifentanil combined with dexmedetomidine was used in the experimental group to compare sedation and pain scores,surgical observation indexes,and incidence of adverse reactions.Results:The sedation score of the experimental group was higher than that of the control group(P<0.05).The incidence of pain score and adverse reactions was lower in the experimental group than in the control group(P<0.05).The operative time and postoperative waking time of the experimental group were shorter than that of the control group,while the intraoperative blood loss of the experimental group was less than that of the control group(P<0.05).Conclusion:Percutaneous nephrolithotripsy using remifentanil and dexmedetomidine general anesthesia,anesthesia effect is significant,analgesic effect is better,not only will reduce intraoperative blood loss,but also reduce the incidence of adverse reactions,clinical application value is high.
