摘要
目的:研究促性腺激素释放激素激动剂(GnRh-a)与左炔诺孕酮宫内缓释系统(LNG-IUS,商品名:曼月乐)联合应用对子宫腺肌病的临床疗效。方法:选择2016年4月至2018年4月于本院就诊治疗的84例子宫腺肌病患者为研究对象,按随机抽签法分为对照组和治疗组两组,每组各42例。对照组接受GnRh-a注射治疗3个月,治疗组在对照组的基础上放置LNG-IUS治疗,放置时间在GnRh-a注射结束后进行。分别记录两组GnRh-a治疗前、LNG-IUS治疗后1个月、LNG-IUS治疗后3个月、LNG-IUS治疗后6个月的痛经程度、子宫体积、月经量评分,比较两组不良反应发生情况。结果:治疗后两组的痛经程度均明显减轻,且治疗后3个月和6个月治疗组痛经评分显著小于对照组(P<0.05),治疗后1个月和3个月两组的子宫体积均较治疗前明显缩小(P<0.05),对照组治疗6个月后的子宫体积与治疗前相比无显著差异(P>0.05),治疗组治疗6个月后的子宫体积显著小于治疗前,且与对照组相比差异有统计学意义(P<0.05)。治疗后两组的月经量评分均较治疗前降低,治疗组在各个时间点的评分均小于对照组,差异有统计学意义(P<0.05)。结论:GnRh-a联合LNG-IUS可提高对子宫腺肌病的临床疗效,更能改善患者痛经情况,减缓子宫体积进行性增大,缓解经量增多症状。
Objective: To study the clinical efficacy of combined application of gonadotropin-releasing hormone agonist (GnRh-a) and levonorgestrelreleasing intrauterine system (LNG-IUS, trade name: Mirena) on adenomyosis. Methods: 84 patients with adenomyosis who were treated in the hospital from April 2016 to April 2018 were selected for the study and were divided into control group and treatment group according to the method of random sampling, with 42 cases in each group. Control group was given GnRh-a injection for 3 months, and treatment group was placed with LNG-IUS (performed after the end of GnRh-a injection) on the basis of control group. The dysmenorrhea degree, uterine volume and menstrual volume score were recorded in the two groups before GnRh-a treatment, at 1 month after LNG-IUS treatment, at 3 months after LNG-IUS treatment and at 6 months after LNG-IUS treatment. The occurrence of adverse reactions was compared between the two groups. Results: The dysmenorrhea degree was significantly reduced in the two groups after treatment, and the dysmenorrhea score in treatment group was significantly lower than that in control group at 3 months and 6 months after treatment (P<0.05), and the uterine volume in the two groups at 1 month and 3 months after treatment was significantly smaller than that before treatment (P<0.05), and the uterine volume at 6 months after treatment in control group was not significantly different from that before treatment (P>0.05), and the uterine volume at 6 months after treatment in treatment group was significantly less than that before treatment, but was not significantly different from that in control group (P<0.05). After treatment, the menstrual volume score in the two groups was lower than that before treatment, and the score in treatment group at each time point was lower than that in control group (P<0.05).
