摘要
目的探讨急性和慢性心力衰竭患者生化指标的检测及其临床意义。方法选取2018年8月至2019年1月赣州市人民医院收治的急性心力衰竭患者(A组)30例和慢性心力衰竭患者(B组)30例,分别检测血清降钙素原(PCT)、髓过氧化物酶(MPO)、白细胞介素-6(IL-6)、脑钠肽(BNP)、C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)和可溶性ST2(sST2)的水平,以及检测左心室射血分数(LVEF)的水平,并对A组LVEF水平与血清CRP、TNF-α、PCT、MPO、sST2、BNP、IL-6水平进行相关性分析。结果A组血清CRP、TNF-α、PCT、MPO、sST2、BNP及IL-6水平均高于B组(均P<0.05),A组LVEF水平低于B组(P<0.05)。相关性分析结果显示,A组LVEF水平与血清CRP、TNF-α、PCT、MPO、sST2、BNP、IL-6水平均呈负相关(r分别为-0.292、-0.498、-0.729、-0.720、-0.521、-0.756、-0.700,均P<0.05)。结论炎症、机械牵张生物标志物检测可有效区分急性和慢性心力衰竭,且其水平高低与病情发展有着密切关系。
Objective To explore the detection of biochemical markers in patients with acute/chronic heart failure and their clinical significance. Methods Serum procalcitonin(PCT),myeloperoxidase(MPO),interleukin-6(IL-6),brain natriuretic peptide(BNP),C-reactive protein(CRP),tumor necrosis factor-alpha(TNF-alpha) and soluble ST2(sST2) levels and left ventricular ejection fraction were measured in 30 patients with acute heart failure(group A) and 30 patients with chronic heart failure(group B) who were admitted to Ganzhou People’s Hospital from August 2018 to January 2019.The correlations between LVEF and serum CRP,TNF-α,PCT,MPO,sST2,BNP and IL-6 levels were analyzed in group A. Results Serum levels of CRP,TNF-α,PCT,MPO,sST2,BNP and IL-6 in group A were higher than those in group B( P<0.05).The LVEF in group A was lower than that in group B( P<0.05).Correlation analysis showed that the LVEF was negatively correlated with serum CRP,TNF-a,PCT,MPO,sST2,BNP and IL-6 levels( r=-0.292,-0.498,-0.729,-0.720,-0.521,-0.756 and -0.700,respectively;P<0.05). Conclusion The detection of inflammation and mechanical stretch biomarkers can effectively distinguish between acute and chronic heart failure.Furthermore,the levels of the biochemical markers are closely related to the progression of disease.
