摘要
目的了解儿童布鲁杆菌病(布病)的临床特征。方法选择2010年1月至2018年10月新疆医科大学第一附属医院收治的儿童布病72例,回顾性分析其病历资料。结果72例患儿中男女比例2.13∶1,以12~<17岁为高发(41.7%),主要发生在农牧区,发病高峰在每年的5—8月。临床表现主要为发热、骨关节疼痛、厌食、不明原因消瘦等;关节损害以膝关节(57.6%)、骶髂、髋关节(27.3%)、腰椎(18.2%)受累较常见,部分患儿是可见支气管炎、轻度贫血、肝损伤等表现。治疗后1例患儿因多脏器衰竭死亡,34例患儿失访,8例复发转为慢性,余均治愈。结论儿童布病多来自农牧区,12~<17岁居多,春夏季高发。对于农牧区来源的不明原因发热、骨关节疼痛、厌食、消瘦患儿,初诊时应注意警惕布病的可能。
Objective To investigate the clinical features of brucellosis in children. Methods A total of 72 cases with childhood brucellosis admitted to the First Affiliated Hospital of Xinjiang Medical University from January 2010 to October 2018 were selected to summarize the clinical data. Results Among 72 cases, the male-to-female ratio was 2.13∶1. The disease highly occurred in children at 12-17 years old (41.7%), in farming and stockbreeding areas, and the incidence peak was from May to August each year. The main clinical manifestations were fever, joint pain, anorexia, unexplained weight loss, etc. Joint injuries were commonly seen in the knee (57.6%), sacroiliac joint (27.3%) and lumbar vertebra (18.2%). Some patients complicated with bronchitis, mild anemia and liver injury. One patient died of multiple organ failure and 34 patients were lost to follow-up. Of the remaining 37 children, 8 had relapse and turned to chronic diseases, and the rest children were cured. Conclusions Patients with childhood brucellosis are mainly from farming and stockbreeding areas, most of them are children at the age of 12-17 with a high incidence in spring and summer. Children come from farming and stockbreeding areas who have unexplained fever, joint pain, anorexia or unexplained weight loss should be considered the possibility of brucellosis.
