摘要
目的探讨影响老年髋部骨折患者围术期发生肺部感染的相关危险因素。方法回顾性选取2010年6月至2017年6月在北京市丰盛中医骨伤专科医院住院的老年髋部骨折(年龄≥70岁)患者576例。根据围术期是否发生肺部感染将576例患者分为两组:发生肺部感染组(n=145),未发生肺部感染组(n=431),比较两组患者临床特点,采用SPSS 20.0软件进行数据处理。组间比较采用χ2检验。对单因素分析有统计学意义的指标进行多因素logistic回归分析,筛选出独立的危险因素。结果单因素分析结果表明年龄偏大[(82.46±5.56)岁比(79.22±6.51)岁,t=-5.381,P=0.000]、贫血(65.51%比31.55%)(t=50.696,P=0.000)、低蛋白血症(55.86%比18.10%)(χ2=75.557,P=0.000)、心力衰竭(44.14%比13.69%)(t=65.242,P=0.000)是老年髋部骨折患者围手术期发生肺部感染的影响因素。多因素logistic回归分析结果表明年龄(OR=1.066,95%CI:1.023~1.110;P=0.002)、低蛋白血症(OR=3.215,95%CI:1.736~5.136;P=0.000)和心力衰竭(OR=3.729,95%CI:2.022~5.534;P=0.000)是导致老年髋部骨折患者围术期发生肺部感染的独立危险因素。结论老年髋部骨折围手术期肺部感染的发生是多种因素共同作用的结果。年龄、低蛋白血症、合并心力衰竭是老年髋部骨折围术期发生肺部感染的独立危险因素。
Objective To explore the related risk factors of perioperative pulmonary infection in elderly patients with hip fracture. Methods From June 2010 to June 2017, 576 patients with hip fracture (age≥70 years) admitted in Beijing Fengsheng Special Hospital of Medical Traumatology and Orthopaedics were selected retrospectively.According to whether perioperative pulmonary infection occurred, 576 patients were divided into two groups: pulmonary infection group (n=145), non-pulmonary infection group (n=431). The clinical features were compared between the two groups.SPSS20.0 software was used for data processing.χ2 test was used for comparison between groups.Logistic regression model was established to screen out independent risk factors. Results Single factor analysis showed that older age [(82.46±5.56)years vs.(79.22±6.51)years, t=-5.381, P=0.000)], anemia (65.51% vs.31.55%, t=50.696, P=0.000), hypoalbuminemia (55.86% vs.18.10%,χ2=75.557, P=0.000) and heart failure (44.14% vs.13.69%, t=65.242, P=0.000) were independent risk factors of perioperative hip fracture. ConclusionThe occurrence of perioperative pulmonary infection in elderly patients with hip fracture is the outcome of combined action of multiple factors.Old age, hypoalbuminemia and heart failure are the independent risk factors for perioperative pulmonary infection.
