摘要
目的探讨双侧上尿路结石类型不同的比例和影响因素。方法研究对象为复旦大学附属上海市第五人民医院构建的泌尿系结石成分数据库(DSCU数据库)中112例双侧上尿路结石患者,结果变量为双侧结石类型不同,预测变量为年龄、性别、血肌酐、肾小球滤过率和尿路感染,采用χ2检验、t检验和Wilcoxon秩和检验等进行统计分析。结果112例患者中男性94例(83.9%),平均发病年龄(54.2±4.0)岁。有14.3%(16/112)的患者双侧结石类型不同,均表现为一侧草酸钙结石、另一侧感染性结石。双侧结石类型不同的患者合并尿路感染的比例为100%,而结石类型相同的患者为70.8%,差异有统计学意义(P<0.05)。年龄、性别、血肌酐和eGFR两组间差异无统计学意义。结论DSCU数据库中双侧上尿路结石类型不同的比例占14.3%,合并尿路感染是结石类型不同的可能因素。
ObjectiveTo investigate the prevalence and risk factors of discordance of stone composition among patients with stones in bilateral upper urinary tracts. MethodsPatients with stones in bilateral upper urinary tracts were collected from Database of Stone Composition in Urolithiasis (DSCU).The outcome variables were defined as discordance of stone types in bilateral upper urinary tracts.The predictive variables included age,gender,serum creatinine,estimated glomerular filtration rate (eGFR) and urinary tract infection.Chi-square test, t test and Wilcoxon rank sum test were employed in the statistical analysis. ResultsA total of 112 patients were enrolled,including 94 males (83.9%),and age of onset was 54.2±4.0 years.Discordance of stone types existed in 14.3%(16/112) patients,all of whom had calcium oxalate on one side and infectious calculus on the other side.Urinary tract infection occurred in 100% of the patients with discordant stone types,and in 70.8% of the patients with identical stone types (P<0.05).There were no statistically significant differences regarding age,sex,serum creatinine and eGFR. ConclusionDiscordance of stone types existed in 14.3% of patients with urolithiasis in bilateral upper urinary tracts in DSCU.Urinary tract infection may be a risk factor of discordance of stone composition.
