摘要
目的讨论显微镜下经腹股沟精索静脉高位结扎术治疗精索静脉曲张的疗效及安全性。方法选择2018年1~7月于我院接受治疗的精索静脉曲张患者68例,按照治疗方式分为对照组(34例,腹腔镜下行精索静脉高位结扎术)与观察组(34例,显微镜下经腹股沟精索静脉高位结扎术);比较两组手术时间、术后总住院时间及术后随访6个月期间并发症情况、复发率。结果观察组手术时间较对照组短,随访期间阴囊水肿发生率明显低于对照组,复发率明显低于对照组,差异有统计学意义(P<0.05);两组患者治疗后精子密度、a+b级精子、精子活率均较治疗前升高(P<0.05);但两组间治疗前、后精液质量各指标比较,差异无统计学意义(P>0.05)。结论显微镜下经腹股沟精索静脉高位结扎术治疗精索静脉曲张的效果显著,且能有效降低患者术后并发症发生率,降低患者复发风险。
Objective To explore curative effects and safety of inguinal high ligation of spermatic vein under the microscope in treatment of varicocele. Methods 68 patients with varicocele who were treated in our hospital from January 2018 to July 2018 were selected and divided into the control group (34 cases,laparoscopic high ligation of spermatic vein) and the observation group (34 cases,inguinal high ligation of spermatic vein under the microscope) according to the treatment.The operative time,total postoperative hospital stay and complications and recurrence rate in postoperative follow-up for 6 months were compared between the two groups. Results The operation time of the observation group was shorter than that of the control group.The incidence of scrotal edema was significantly lower than that of the control group during the follow-up period. The recurrence rate was significantly lower than that of the control group. The differences were significant (P < 0.05).The sperm density,a+b grade sperm and sperm motility after treatment of the two groups were higher than those before treatment (P < 0.05).There was no significant difference in the quality of semen before and after treatment between the two groups (P > 0.05). Conclusion Inguinal high ligation of spermatic vein under the microscope in treatment of varicocele has significant curative effects.It can effectively reduce the incidence of postoperative complications and reduce the risk of recurrence.
