摘要
目的探讨荧光原位杂交(FISH)技术及染色体核型分析(G显带)在检测儿童急性白血病(AL)中的临床价值。方法回顾性分析2017年6月至2019年5月在河南省儿童医院血液科收治的158例AL患儿的临床资料,分别采用FISH技术及染色体G显带技术对患儿骨髓标本进行检测。分析AL患儿骨髓染色体核型构成及其异常结果,比较FISH与染色体G显带检出遗传学异常情况。结果染色体G显带一共检出异常43例,正常80例,培养失败、未见分裂相35例,实验成功率为77.8%(123/158),异常检出率为35.0%(43/123);FISH共检出阳性87例,阳性检出率为55.1%(87/158),阴性71例,其中白血病基因阴性但在计数过程中发现异常信号12例。FISH检测基因阳性率高于染色体G显带(P<0.05)。结论 FISH技术异常检出率高于显带技术。两者结合能够使遗传学异常的检测更准确,在儿童AL的诊断和分型中具有较高的临床价值。
Objective To explore the clinical value of fluorescence in situ hybridization (FISH) and chromosome karyotype analysis (G-banding) in the detection of childhood acute leukemia (AL). Methods The clinical data of 158 children with AL admitted in hematology department of Henan Children's Hospital from June 2017 to May 2019 were retrospectively analyzed, and FISH and G-banding techniques were used to detect bone marrow samples of children. The karyotype composition and abnormal results of bone marrow chromosomes in children with AL were analyzed. The genetic abnormalities detected by FISH and G-banding were compared. Results There were 43 cases of abnormal chromosome Gbanding, 80 cases of normal, and 35 cases of culture failure with no mitotic phase. The success rate of the experiment was 77.8%(123/158) and the abnormal detection rate was 35.0%(43/123). A total of 87 cases were positive for FISH, the positive detection rate was 55.1%(87/158), and 71 cases were negative, of which 12 cases were found abnormal signal during the counting process. The positive rate of FISH detected genes was higher than that in chromosome Gbanding (P<0.05). Conclusion The abnormal detection rate of FISH technique is higher than that of banding technique. The combination of the two methods can make the detection of genetic abnormalities more accurate, with high clinical value in the diagnosis and classification of AL in children.
