摘要
目的探讨猫抓病的临床表现和病理特点,掌握猫抓病的诊断、鉴别诊断及治疗方法,从而提高其诊断及治疗水平。方法回顾性分析5例猫抓病患者的临床资料,观察经形态学确定为猫抓病的石蜡包埋淋巴结组织,使用银染色法进行病原体的检测,并复习文献。结果 5例猫抓病患者均有与猫密切接触史或者抓、咬伤史,临床表现为淋巴结肿大,形态学表现为淋巴结内肉芽肿形成,银染色法查见成点、成线、成簇黑色短小杆菌,5例临床诊断均未考虑到猫抓病。结论猫抓病易误诊,注意询问患者的猫接触史或抓、咬伤史,结合镜下形态学表现可确定诊断。因此,对局部浅表淋巴结增大者,鉴别诊断中一定要考虑到猫抓病。
Objective To explore the clinical manifestations and pathological characteristics of cat scratch disease, and to master the diagnosis, differential diagnosis and treatment, so as to improve the level of clinical diagnosis and treatment.Methods The clinical data of 5 cases of cat scratch disease were retrospectively analyzed, the paraffin-embedded lymph node tissue identified by morphology as cat scratch disease was observed, the pathogen was detected by silver staining and the literature was reviewed. Results All the five patients had a history of close contact, scratches or biteswith cats. The clinical manifestations were enlarged lymph nodes, the morphological manifestations were granuloma formation in lymph nodes, and silver staining showed spots, lines and clusters of black short bacilli. Cat scratch disease was not considered in the clinical diagnosis of 5 cases. Conclusion Cat scratch disease is easy to be misdiagnosed. The diagnosis can be confirmed by asking the patient about the history of close contact, scratches or biteswith cats combined with the morphological manifestations under the microscope. Therefore, cat scratch disease must be considered in differential diagnosis of local superficial lymph node enlargement.
