摘要
目的:观察益气活血胶囊治疗高血压早期肾损害的临床疗效及对血管内皮功能的影响。方法:选取气虚血瘀型高血压早期肾损害患者60例,随机分为两组各30例,两组治疗均给予贝那普利,或加用氨氯地平,治疗组在此基础上加用益气活血胶囊,疗程均为8周,观察比较治疗前后两组血清中性粒细胞明胶酶相关脂质运载蛋白NGAL、血清胱抑素C(CysC)、内生肌酐清除率(GFR),及血浆一氧化氮(NO)、内皮素(ET-1)和血管性假血友病因子(VWF)水平的变化。结果:治疗后,两组NGAL、CysC水平均低于治疗前(P﹤0.05),且治疗组优于对照组(P﹤0.05);两组GFR均较治疗前有所上升(P﹤0.05),两组相比未见明显差异(P﹥0.05)。治疗后,两组ET-1、VWF水平均较治疗前下降,NO水平较治疗前上升(P﹤0.05);两组相比,治疗组优于对照组(P﹤0.05)。两组中医证候疗效比较,治疗组疗效更佳(P﹤0.05)。结论:益气活血胶囊能改善高血压早期肾损害,改善患者的中医证候,对血管内皮功能具有保护作用。
Objective:To observe the clinical effect of Yiqi Huoxue capsule on hypertensive early renal damage and the influences on vascular endothelial function.Methods:60 cases of selected patients with early renal damage due to hypertension of qi deficiency and blood stasis syndrome were randomly divided into two groups,30 cases in each geoup.Both groups were treated with benazepril,or plus amlodipine.The treatment group was treated plus with Yiqi Huoxue capsule on this basis.For both groups,the course of treatment was 8 weeks.The changes of levels of neutrophil gelatinase-associated lipocalin(NGAL),serum cystatin C(Cys C)and endogenous creatinine clearance(GFR)in serum as well as nitric oxide(NO),endothelin-1(ET-1)and von willebrand factor(VWF)in plasma were observed and compared before and after treatment in two groups.Results:After treatment,the levels of NGAL and Cys C in both groups were lower than before treatment(P﹤0.05),and the treatment group was better than control group(P﹤0.05);GFR of both groups increased somewhat compared with before treatment(P﹤0.05),and compared between two groups,there was no significant difference(P﹥00.05);the levels of ET-1 and VWF in both groups were lower than before treatment,the level of NO was higher than before treatment(P﹤0.05),and compared between groups,the treatment group was better than control group(P﹤0.05).Compared the therapeutic effects of TCM syndromes between two groups,the therapeutic effect of treatment group was better(P﹤0.05).Conclusion:Yiqi Huoxue capsule can improve the hypertensive early renal damage and TCM syndromes of patients,and has protective effect on vascular endothelial function.
