掌侧经皮埋头空心加压螺钉内固定微创治疗新鲜腕舟骨腰部骨折 被引量:7

Volar approach percutaneous minimally invasive treatment for fresh scaphoid waist fracture with headless hollow compression screw

摘要 目的探讨经皮埋头空心加压螺钉内固定治疗新鲜舟骨腰部骨折的手术技巧及临床效果。方法自2015年5月至2018年5月,我们应用掌侧入路经皮埋头空心加压螺钉内固定治疗新鲜腕舟骨腰部骨折患者14例。其中Herbert A2型3例、Bl型2例、B2型9例,均为无移位或微小移位(≤1mm)骨折。术后进行病例随访,评估临床疗效。结果本组14例手术均顺利完成,术中出血5-20ml,平均12ml;手术时间40-75min,平均55min;骨折复位良好,螺钉位置恰当。术后13例获得随访,失访1例,随访时间为6-18个月,平均12个月,骨折均愈合良好,未见内固定物松动。根据Mayo腕关节功能评分评估疗效:优10例,良2例,可1例。结论采用掌侧经皮埋头空心螺钉内固定微创治疗新鲜舟骨腰部骨折,损伤小,骨折愈合率高,疗效确切。 Objective To explore the operation skills and clinical efficacy of volar approach percutaneous minimally invasive treatment for fresh scaphoid waist fracture with headless hollow compression screw. Methods From May 2015 to May 2018, 14 cases of fresh scaphoid waist fractures were treated with percutaneous headless hollow compression screws through the volar approach. There were 3 cases of Herbert A2 type, 2 cases of Bl type and 9 cases of B2 type. They were undisplaced or slightly displaced (≤1 mm) fractures. The patients were followed up after operation to evaluate the clinical efficacy. Results All the 14 cases were successfully completed with intraoperative bleeding of 5 to 20 ml with an average of 12 ml. The operation time ranged from 40 to 75 min with an average of 55 min. The fracture reduction and screw position were correct. 13 patients were followed up and 1 patient was lost. The follow-up time ranged from 6 to 18 months, with an average of 12 months. All the fractures healed well without loosening of internal fixator. According to the Mayo wrist function score, the results were rated as excellent in 10 cases, good in 2 cases and fair in 1 case. Conclusion The volar approach percutaneous minimally invasive treatment for fresh scaphoid waist fracture with headless hollow compression screw has the advantages of small injury, high rate of fracture healing and definite curative effect.
