摘要
目的探讨自体骨联合同种异体骨在胸腰椎爆裂骨折患者单侧椎弓根植入后的应用效果。方法回顾性分析80例胸腰椎爆裂骨折患者的临床资料,根据术中植入材料的不同将其分为联合组(自体骨联合同种异体骨,池=47)与对照组(同种异体骨,池=33)比较2组的治疗效果。结果术后,2组椎体前缘压缩率均较术前显著提高(P<0.05),但2组间术后8周及6个月的椎体前缘压缩率均无显著差异(P>0.05);术后8周,2组脊椎矢状位CoUb角均较术前显著下降,术后6个月较术后8周小幅上升,组内不同时点差异显著(P<0.05),且术后6个月时联合组CUb角显著小于对照组(P<0.05);术后8周,2组椎管占位率均较术前显著降低,术后6个月椎管占位率小幅上升,组内不同时点差异显著(P<0.05),且联合组术后8周及6个月的椎管占位率均显著低于对照组(P<5.55),植骨融合率均显著高于对照组(P<5.55);术后6个月,2组神经功能均较术前显著改善(P<5.05),且联合组术后6个月的神经功能显著优于对照组(P<5.05)o结论自体骨联合同种异体骨在爆裂性胸腰段骨折中具有良好的应用价值,能促进伤椎的恢复。
Objechvv To study thd effect of autolouous boad combiaed with allouedeia boad in rnilaterai venedrai pedicic arch implantatiou in patiecis with thoracolumban bunt fractrres.Methodt The clinicai iata of 80 patiedit with thoracolumbar burst fractures in thd Oospitai were retrospeahveiy analyzed.The patiedis were iiviCed into group(aatolouous boud with allugedeic boud,n=47)ani coutrou group(allouedeic bone,n=33)accornina to thd yffenat intraouerativv implantatioa matenais.The treatmedi effecis were compared betweed the twogroups.Resultt A8cc spraeo,We compressiou raia of antenoc venedrai body wus sicnificantiz iacreased,ani tha dimeredco wns sianimcaai ni ymeredi tima points(P<0.05).Howcvcc,there were eu sianimcaai dimeredcas in the compressiou rate of antenoc venedrai body ni 1 weed ani 6 mouths after spraeo betweee combieed goup ani control group(P>0.05).At 1 weed afteo spraeo,the Cobb aneie of spinai saaittai positiou in the two goups wap sicnificontiz lower thaa thai before spoy,ni wap increesed motiy ai 6 mouths afteo spraeo compared with thai ai 8 weed afteo spraeo(P<0.03),ant the CoOb aneie in combied group was sicnificontiz lower than thai in coutol goup ai 6 mouths after spraeo(P<0.05).At 1 weed aftae spraeo,the spinai cdiai occopncy rate in the two goups was sionificantiz lower than thai in the sama group before spraeo,ani was increased months 6 mouths after spraeo(P<0.05),ani there was a significani betweed-goup dimerenco in spinai canai occupancy rate(P<0.05).The spinal canal occupancy rate in combined group at 1 wek and 6 months after suody was sinnincuntly lowda thna that in control group(P<0.05).Thd fpsiou ratd of boad graftina in combiaed group was sinnificuntry higgro thna thal in control group al 1 weed and 6 months aftra sproery(P<0.05).At 6 mouths aftro sproero,the dedrolonicyl fpdctiou in the two groups was sinnificyntly imprcued thnd before suroery(P<0.05),ant the combined group wns sinnificontly higger that thd cyutrol group at 6 mouths aftdo spraero(P<0.05).Conclusion Autolonous botd combited
