摘要
目的:探讨腰俞穴麻醉结合埋药线用于混合痔围术期的镇痛效果。方法:选取2017年4月至2018年3月海南省中医院肛肠科收治的拟行混合痔手术患者120例作为研究对象,以随机数字表法分为观察组、对照1组及对照2组,每组40例。观察组患者给予腰俞穴麻醉结合腰俞穴埋药线方式进行镇痛,对照1组患者给予腰俞穴麻醉结合腰俞穴埋羊肠线方式进行镇痛,对照2组患者给予腰俞穴麻醉结合长强穴埋药线进行镇痛。比较三组患者的麻醉效果、视觉模拟评分(visual analogue scale,VAS)和不良反应发生情况的差异。结果:观察组、对照1组和对照2组患者的麻醉总有效率分别为97.50%(39/40)、92.50%(37/40)和95.00%(38/40),三组的差异均无统计学意义(P>0.05)。术后6、12、24、48及72h,观察组患者的VAS评分明显低于对照1组、对照2组,差异均有统计学意义(P<0.05);对照1组与对照2组患者术后各时点VAS评分的差异均无统计学意义(P>0.05)。观察组患者尿潴留、排便困难及睡眠障碍的发生率明显低于对照1组、对照2组,差异均有统计学意义(P<0.05);对照1组与对照2组患者不良反应发生率的差异无统计学意义(P>0.05)。结论:腰俞穴麻醉结合埋药线用于混合痔围术期患者的镇痛效果更好,且不良反应少,安全可靠。
OBJECTIVE:To probe into the analgesic effect of lumbar acupoint anesthesia combined with medicated thread embedding in perioperative period of mixed hemorrhoids.METHODS:120 patients undergoing surgery of mixed hemorrhoids admitted into proctology department of Hainan Hospital of Traditional Chinese Medicine from Apr.2017 to Mar.2018 were selected and divided into observation group,control group 1 and control group 2 via random number table,with 40 cases in each group.The observation group was given lumbar acupoint anesthesia combined with medicated thread embedding in lumbar acupoint,the control group 1 was given lumbar acupoint anesthesia combined with catgut embedding in lumbar acupoint,the control group 2 was given lumbar acupoint anesthesia combined with catgut embedding in Changqiang acupoint.Differences in anesthetic effect,VAS scores and incidence of adverse drug reactions among three groups were compared.RESULTS:The total effective rates of anesthesia of observation group,control group 1 and control group 2 were respectively 97.50%(39/40),92.50%(37/40) and 95.00%(38/40),the difference had no statistical significance ( P >0.05).At 6 h,12 h,24 h,48 h and 72 h after surgery,the VAS scores of observation group were significantly lower than those of control group 1 and control group 2,with statistically significant differences ( P <0.05);the difference of VAS scores at each time-points between control group 1 and control group 2 had no statistical significance ( P >0.05).The incidences of uroschesis,difficult defecation and sleep disorders of observation group were significantly lower than those of control group 1 and control group 2,with statistically significant differences ( P <0.05);there was no statistical significance in difference of incidences of adverse drug reactions between control group 1 and control group 2 ( P >0.05).CONCLUSIONS:The combination of lumbar acupoint anesthesia and medicated thread embedding has better analgesic effect for patients in perioperative period of mixed hemorrhoids,with fe
