摘要
目的探讨脉压指数(PPI)与脑血流动力学(CVHI)积分值的相关性。分析不同PPI水平与脑卒中相关危险因素之间的关系。方法2018年1-12月,以在新疆医科大学第一附属医院完成CVHI检测的21 316例体检人群为研究对象。同时测量其身高、体质量、血压并对其脑卒中相关危险因素进行调查。根据PPI不同水平将体检人群分为A组(PPI<0.40)、B组(PPI 0.41~0.50)和C组(PPI>0.51)。分析不同PPI水平与CVHI积分值之间的相关性以及PPI水平与脑卒中相关危险因素之间的关系。结果在PPI分组中,不同性别、民族和年龄的人群构成比差异均有统计学意义(P<0.05)。B组和C组中女性占比高于男性,年龄>50岁组占比明显高于其他年龄组,其比例随年龄的增长呈上升趋势;B组和C组高血压病史、高血脂病史、糖尿病病史及吸烟、饮酒人群构成比高于A组(PPI<0.40);3组人群CVHI正常率(>75分)分别为90.7%、86.5%和61.1%,依次呈下降趋势,但在高危、中危、低危人群中则呈上升趋势;PPI与CVHI存在相关性(r=-0.285,P<0.05)。当PPI为0.41~0.50时,PPI与CVHI存在相关性(r=-0.295,P<0.05);PPI>0.51时,PPI与CVHI呈明显负相关(r=-0.660,P<0.05)。结论PPI水平越高CVHI积分值越低,当PPI>0.50时,应予以高度重视。
Objective To explore the correlation between pulse pressure index(PPI) and cerebral hemodynamics(CVHI) score and analyze the relationship between PPI levels and stroke risk factors. Methods From January 2018 to December 2018, a total of 21316 patients who had completed CVHI test in the First Affiliated Hospital of Xinjiang Medical University were selected as study objects. Their height, weight and blood pressure were measured and their risk factors for stroke were investigated. According to different levels of PPI, the subjects were divided into group A with PPI<0.40, group B with PPI 0.41-0.50, and group C with PPI>0.50. The correlation between PPI and CVHI scores was investigated. And the association between PPI levels and stroke related risk factors was analyzed. Results In PPI groups, there were significant differences in the proportion of people of different genders, ethnics and ages(P<0.05). The proportion of female in group B and C were higher than that of male, and the proportion of those aged over 50 was significantly higher than that of other age groups. The proportion of female in group B and C increased with age. The proportion of systolic blood pressure, hypertension, hyperlipidemia, diabetes, smoking and drinking population in group B and C were higher than those in group A(PPI<0.40). The normal rate of CVHI in the three groups(>75 points) were 90.7%, 86.5% and 61.1% respectively, showing a downward trend in turn, but an upward trend in high-risk, medium-risk and low-risk groups. PPI was correlated with CVHI(r=-0.285, P<0.05). When PPI was 0.41-0.50, there was a correlation between PPI and CVHIV(r=-0.295, P<0.05);when PPI was more than 0.51, PPI was negatively correlated with CVHIV(r=-0.660, P<0.05). Conclusion PPI is negatively correlated with CVHI. When PPI>0.50, it should be paid great attention to.
