硫代嘌呤甲基转移酶基因多样性对急性淋巴细胞白血病患儿使用糖皮质激素耐受性的影响 被引量:5

The influence of single nucleotide polymorphisms of TPMT on the toxicity of glucocorticoids in children with acute lymphoblastic leukemia

摘要 目的探究硫代嘌呤甲基转移酶(thiopurine methyltransferase,TPMT)基因多态性对于急性淋巴细胞白血病(acute lymphoblastic leukemia,ALL)患儿使用糖皮质激素(glucocorticoids,GCs)所致副作用的影响。方法对245例ALL患儿和210例对照组患儿进行TPMT基因的3个SNPs(G238C,G460A和A719G)进行分析,并根据常见毒性标准(NCI-CTC)评估GCs相关毒性及其与基因型的关联。结果感染发生率与突变基因型A719G之间观察到有相关性(P=0.021),并且是其GCs后发生感染的独立危险因素(P=0.044);G460A与肝毒性之间有显著相关性,但不是独立危险因素。结论TPMT基因的A719G与糖皮质激素的使用导致儿科ALL治疗期间发生感染有关,儿童ALL治疗前应该进行基因检测。 Objective To evaluate the influence of TPMT gene polymorphism on the side effects caused by glucocorticoids in children with acute lymphocytic leukemia(ALL).Methods Three single nucleotide polymorphisms(SNPs)of the TPMT gene(G238C,G460A and A719G)were analysed in 245 children with ALL and 210 control subjects.Participants were treated according to the ALL 2000 study protocol.Toxicities were assessed and associations with genotypes were evaluated according to Common Toxicity Criteria(NCI-CTC).Results A significant correlation was observed between the incidence of infection and the mutant genotype A719G(P=0.021)and was an independent risk factor for infection after GC(P=0.044);There was a significant correlation between G460A and hepatotoxicity,but G460A was not an independent risk factor.Conclusions The use of TPMT gene A719G and glucocorticoids is associated with infection during pediatric ALL treatment,and genetic testing should be performed before treatment for children with ALL.
