不同运动方式对糖尿病前期患者糖代谢影响的网状meta分析 被引量:6

A network meta-analysis of the effect of different exercise methods on glucose metabolism inpatients with prediabetes mellitus

摘要 目的:采用网状meta分析系统性评价不同运动方式对糖尿病前期(PD)患者糖代谢的影响效果。方法:计算机检索The Cochrane Library、PubMed、Web of Science、Embase等数据库关于不同方式运动干预PD患者的随机对照试验(RCT),纳入运动对PD患者糖代谢影响的RCT,采用R软件联合gemtc软件进行统计学分析,网状meta分析评价不同运动方式对PD患者糖代谢的影响。结果:最终纳入21项研究,共1 590例PD患者。网状Meta分析结果显示,有氧、抗阻及联合运动较无运动干预组均可降低空腹血糖(FPG)、糖化血红蛋白(HbA1c)及胰岛素抵抗(HOMA-IR)(P<0.05),而对于餐后2 h血糖(2 h PG),有氧、抗阻运动改善效果与无运动干预组比较,差异有统计学意义(P<0.05);概率排序图显示,有氧运动成为改善2 h PG最优方式的概率最大,在降低FPG、HbA1c,抗阻运动成为最优运动方式的概率最大,而联合运动成为改善HOMA-IR最佳方式的概率最大。结论:有氧、抗阻及联合运动均可改善PD患者的糖代谢。 Objective:To evaluate the effect of different exercise patterns on glucose metabolism in patients with pre-diabetes(PD)mellitus by network meta-analysis.Methods:The Cochrane Library,PubMed,Web of Science,Embase and other databases were searched by computer for randomized controlled trials(RCTS)of PD patients with different types of exercise.RCTs about the effect of exercise on glucose metabolism were included,statistically analyzed by gemtc and R.Meta-analysis was used to evaluate the effect of different exercise patterns on glucose metabolism in PD patients.Results:A total of 21 researches were included,including 1,590 PD patients.The results of network meta-analysis showed that aerobic exercise,resistance exercise and combined exercise could all reduce FPG,HbA1c and HOMA-IR(P<0.05),while for the improvement of 2 h blood glucose(2 h PG),there were significant differences between aerobic exercise,resistance exercise group and no exercise group(P<0.05).According to the probability sequence diagram,the resistance exercise has the highest probability to become the optimal exercise modefor improve FPG and HbA1c,the aerobic exercise has a more significant effect on the reduction of 2 h PG,and the combined exercise has the highest possibility to become the optimal exercise mode to reduce HOMA-IR.Conclusion:Based on the results of network analysis and probability sequence diagram,the results of this study showed that aerobic exercise,resistance exerciseandcombined exercisecan significantly improve the glucose metabolism of PD patients.
