冠状动脉粥样硬化性心脏病患者体内组织因子水平与血脂的相关性分析 被引量:12

摘要 目的探讨冠状动脉粥样硬化性心脏病(CHD)患者体内组织因子水平与血脂各指标的相关性。方法入选2013年7月至2014年3月因胸痛到首都医科大学附属北京安贞医院就诊并收入院的患者210例。入院后根据冠状动脉造影检查结果分为CHD组(120例)和对照组(90例)。CHD组分为急性冠状动脉综合征(ACS)亚组(31例)和稳定型CHD亚组(SCHD亚组,89例)。收集临床资料,采集空腹静脉血检测血常规、血生化等各项指标。结果 CHD组患者血清组织因子水平与中性粒细胞计数、高敏C反应蛋白(hs-CRP)水平呈正相关(r=0. 232,P=0. 011;r=0. 571,P=0. 012)。多因素Logistic回归分析结果显示糖尿病、低密度脂蛋白胆固醇(LDL-C)、hs-CRP是CHD的独立危险因素(均P <0. 05)。CHD组和对照组2组数据相比,组织因子水平差异无统计学意义(P> 0. 05)。ACS亚组、SCHD亚组、对照组3组间比较,ACS亚组患者组织因子水平高于SCHD亚组及对照组[(129±23) ng/L比(116±20)、(114±36) ng/L],差异有统计学意义(P <0. 05)。在ACS亚组患者中,LDL-C水平和组织因子水平呈正相关(r=0. 609,P=0. 001)。结论 ACS患者LDL-C水平与组织因子水平呈正相关,提示LDL-C、组织因子可能与动脉粥样硬化斑块的破裂相关。 Objective To explore the relationship between tissue factor( TF) and blood lipid levels in patients with coronary atherosclerotic heart disease( CHD). Methods A total of 210 patients who visited Beijing Anzhen Hospital,Capital Medical University because of chest pain were enrolled from July 2013 to March 2014.According to coronary angiographic findings after admission,the patients were divided into CHD group( 120 cases)and control group( 90 case). The CHD group was divided into acute coronary syndrome( ACS) group( 31 cases)and stable CHD( SCHD) group( 89 cases). Clinical data including fasting blood routine and biochemical tests were collected. Results Serum TF level was positively correlated with neutrophil count and high sensitivity C-reactive protein( hs-CRP) level in CHD patients( r = 0. 232,P = 0. 011;r = 0. 571,P = 0. 012). Multivariate logistic regression showed that diabetes,low density lipoprotein cholesterol( LDL-C) and hs-CRP were independent risk factors of CHD( all P < 0. 05). TF level in ACS group was significantly higher than that in SCHD group and control group[( 129 ± 23) ng/L vs( 116 ± 20),( 114 ± 36) ng/L]( P < 0. 05). In ACS group,LDL-C level was positively correlated with TF level( r = 0. 609,P = 0. 001). Conclusion Serum LDL-C level is positively correlated with TF level in ACS patients,suggesting that LDL-C and TF may be involved in atherosclerotic plaque rupture.
