摘要
目的探讨尿沉渣白细胞及细菌定量计数在尿路感染诊断中的应用价值。方法选取2018年1月至2018年12月大同市第四人民医院收治的200例疑似尿路感染患者,采集所有患者的尿液标本,均行尿沉渣白细胞及细菌定量计数、尿液细菌培养,以尿细菌培养结果为"金标准",分析尿沉渣白细胞及细菌定量计数诊断尿路感染的价值。结果尿细菌培养阳性检出率与尿沉渣检测比较,差异未见统计学意义(P>0.05);以尿细菌培养结果为"金标准",尿沉渣白细胞及细菌定量计数阳性符合率为91.67%,两种检测方法具有良好的一致性(Kappa=0.813);尿沉渣白细胞及细菌定量计数诊断尿路感染的灵敏度为93.22%,特异度为87.80%,准确度为91.00%。结论尿沉渣白细胞及细菌定量计数在诊断尿路感染上具有操作简便、成本低、阳性符合率高等优点,可作为临床快速筛查尿路感染的一种重要方式。
Objective To investigate the application value of quantitative count of leukocytes and bacteria in urinary sediment in the diagnosis of urinary tract infection. Methods A total of 200 patients with suspected urinary tract infection admitted to the Fourth People’s Hospital of Datong from January 2018 to December 2018 were selected. Urine specimens were collected from all patients. Urine sediment leukocytes and bacteria were counted quantitatively, and urine bacteria were cultured. The value of urinary sediment leukocyte count and bacterial quantitative count in diagnosis of urinary tract infection was analyzed with the results of urinary bacterial culture as "gold standard". Results The positive detection rate of urine bacterial culture did not significantly differ from that of urine sediment determination (P>0.05). With the results of urinary bacterial culture as the "gold standard", the positive coincidence rate of quantitative count of white blood cells and bacteria in urine sediment was 91.67%, and the two detection methods had a consistency (Kappa=0.813). The sensitivity, specificity and accuracy of urinary sediment white blood cells and bacterial quantitative counting in diagnosis of urinary tract infection were 93.22%, 87.80% and 91.00% respectively. Conclusions The quantitative count of white blood cells and bacteria in urinary sediment has the advantages of simple operation, low cost and high positive coincidence rate in the diagnosis of urinary tract infection.
