摘要
目的观察香远合剂联合左甲状腺素钠片治疗桥本甲状腺炎(HT)合并甲状腺功能减退(甲减)患者的临床疗效。方法选取HT合并甲减患者100例分为实验组和观察组,每组各50例,实验组和观察组统称为治疗组,另收集年龄匹配的健康者40例为对照组。收集所有研究对象的一般资料及生化指标,行甲状腺超声检查。观察组单用左甲状腺素钠片治疗,实验组在观察组基础上联合香远合剂10mL/次,3次/d,实验组和观察组口服相应剂量的左甲状腺激素片调整甲状腺激素及促甲状腺激素在正常水平。对照组不给予药物治疗。3个月后检测并记录实验组及观察组甲状腺体积、甲状腺相关抗体[抗甲状腺球蛋白抗体(TgAb)、抗甲状腺过氧化物酶抗体(TPOAb)、促甲状腺激素受体抗体(TRAb)]、促甲状腺激素(TSH)、游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、血沉(ESR)、C反应蛋白(CRP)、补体C3、补体C4水平。结果共有92例(92.00%)患者完成3个月的治疗,其中实验组45例,观察组47例;治疗后实验组TSH、甲状腺体积、ESR水平低于治疗前(P<0.05),FT3、FT4高于治疗前(P<0.05);治疗后实验组甲状腺体积低于观察组(P<0.05)。结论香远合剂联合左甲状腺素钠片治疗HT合并甲减患者,能有效缩小甲状腺体积,对临床治疗有一定的参考价值。
Objective To investigate the clinical efficacy of xiangyuan mixture combined with levothyroxine sodium tablets in the treatment of hashimoto thyroiditis (HT) combined with hypothyroidism (hypothyroidism). Methods100 patients with HT combined with hypothyroidism were selected as the study subjects,and were divided into experimental group and observation group by random number table method,with 50 patients in each group.40 healthy controls were collected as the control group. General data and biochemical indexes of all subjects were collected,and thyroid ultrasonography was performed.The observation group was treated with levothyroxine sodium tablets alone,and the experimental group was treated with10 mL xiangyuan mixture three times a day on the basis of the observation group.Patients in the experimental group and the observation group were given orally the corresponding dose of levothyroxine tablets to adjust the levels of thyroid hormone and thyrotropin in the normal range.The control group was not given medication.3 months later,the thyroid volume and thyroid-related antibodies (anti- thyroglobulin antibody (TgAb),anti-thyroid peroxidase antibody (TPOAb),thyroid stimulating hormone receptor antibody (TRAb),Thyroid stimulating hormone (TSH),free triiodothyronine (FT3),free thyroxine (FT4),erythrocyte sedimentation rate (ESR),C-reactive protein (CRP),complement C3,complement C4 levels were detected and recorded and statistically analyzed in the experimental group and the observation group. Results A total of 92 patients (92%) completed the 3-month treatment,including 45 patients (90%) in the experimental group and 47 patients (94%) in the observation group.After treatment,the TSH,thyroid volume and erythrocyte sedimentation level were lower than those before treatment ( P <0.05),FT3 and FT4 were higher than before treatment ( P <0.05).After treatment,thyroid volume in the experimental group was lower than that in the observation group ( P <0.05).
