摘要
目的 探讨综合术前护理对膀胱癌根治加尿流改道术患者的影响。方法 分析本院2016年1月至2018年5月行膀胱癌根治加尿流改道术的患者共82例,其中对照组40例,观察组42例。对照组行常规护理,观察组行综合术前护理。比较两组术后切口感染率、泌尿系感染率、造口周围皮肤炎率、肺部感染率及入院前后焦虑评分等指标。结果 两组患者的切口感染率(14.3%比10.0%)及泌尿系感染率(11.9%比17.5%)比较,差异均无统计学意义;观察组的造口周围皮肤炎率(4.8%比22.5%)及肺部感染率(7.1%比27.5%)均低于对照组,差异均有统计学意义(均P<0.05);两组出院时的焦虑评分均较入院时有改善,观察组出院时的焦虑评分较对照组改善更明显([31.52±12.36)分比(46.69±9.92)分],差异有统计学意义(P<0.05)。结论 在膀胱癌根治术加尿流改道后患者中进行综合术前护理,能有效减少部分术后并发症和改善患者焦虑情绪。
Objective To investigate the effect of preoperative comprehensive care for patients with bladder cancer treated by radical cystectomy and urinary diversion. Methods A total of 82 patients with bladder cancer treated by radical cystectomy and urinary diversion in our hospital from January, 2016 to May, 2018 were enrolled, including 40 patients in the control group and 42 patients in the observation group. The control group received routine nursing, and the observation group underwent preoperative comprehensive care. The incision infection rate, urinary tract infection rate, dermatitis rate around the stoma, lung infection rate, and anxiety score before admission and after discharge were compared between the two groups. Results There were no statistical differences in incision infection rate (14.3% vs. 10.0%) and urinary tract infection rate (11.9% vs. 17.5%) between the two groups. The dermatitis rate around the stoma (4.8% vs. 22.5)%) and lung infection rate (7.1% vs. 27.5%) were lower in the observation group than the control group (both P < 0.05). The anxiety scores at the time of discharge were improved compared with those at admission, and the anxiety score at the time of discharge was significantly lower in the observation group than in the control group [(31.52±12.36) vs.(46.69±9.92), P < 0.05]. Conclusion Preoperative comprehensive care for patients undergoing radical surgery and urinary diversion for bladder cancer can reduce some postoperative complications and improve their anxiety.
