罗伊氏乳杆菌辅助治疗种植体周围炎的临床和微生物学疗效 被引量:2

Clinical and microbiological effects of Lactobacillus reuteri as adjuvant therapy for peri-implantitis

摘要 目的探究罗伊氏乳杆菌辅助治疗种植体周围炎(PIS)的临床疗效,并分析治疗后种植体周围的微生物环境。方法86例2016年12月-2018年12月于我院就诊的PIS患者,按照随机数字表法将所有患者分为常规组(n=43)和联合组(n=43)。所有患者均给予一次牙周基础治疗及种植体清洗治疗,而联合组在此基础上增加口服拜奥益生菌滴剂(主要成分为罗伊氏乳杆菌),共计治疗90d。而后于治疗前后分别对比两组患者出血指数(BI)、菌斑指数(PLI)、种植体临床冠高度、种植体探诊深度以及以上4组数据于两个时间点间的差值。同时,取患者种植体周围口袋中分泌物,并分析其治疗前后龈沟液中细胞因子间的差异;根据以往经验,采用real-timePCR技术分析标本中常见致病菌的负荷量。结果联合组治疗后BI显著低于常规组(P<0.05),两组患者治疗前后BI、PLI及种植体周围探诊深度等指标比较差异具有统计学意义(均P<0.05);联合组治疗后龈沟液中IL-1β、IL-6、TNF-α和MMP-8均显著低于常规组(均P<0.05);联合组治疗前后福赛斯拟杆菌及牙龈卟啉单胞菌负荷量差异具有统计学意义(均P<0.05)。结论罗伊氏乳杆菌辅助治疗PIS能更有效缓解患者症状,同时调节种植体周围细胞因子及微生物环境,对PIS有较好的治疗作用。 Objective To explore the clinical efficacy of Lactobacillus reuteri in the treatment of peri-implantitis(PIS)and analyze the microbial environment around the implant after treatment. Methods Eighty-six patients with PIS who were admitted to our hospital from December 2016 to December 2018 were randomly divided into the routine group(n=43)or the combination group(n=43).All the patients were given a basic periodontal treatment and implant cleaning,while those in the combination group were additionally give BioGaia probiotic drops(Lactobacillus reuteri)orally for 90 days.The bleeding index(BI),plaque index(PLI),clinical crown height of implant,probing depth of implant before and after treatment and the changes in the above four groups of data after treatment were compared between the two groups.Specimens of secretions were collected from the pockets around implants,and the cytokines in gingival crevicular fluid before and after treatment were analyzed.Real-time PCR was used to analyze the load of pathogenic bacteria in the specimens. Results After treatment,the BI in the combination group was significantly lower than that in the routine group.There were statistically significant differences in the loads of Bacteroides forsythus and Porphyromonas in the combination group before and after treatment(all P<0.05).The differences in the BI,PLI and probing depth around the implant before and after treatment were statistically significant(all P<0.05).The levels of IL-1β,IL-6,TNF-α and MMP-8 in gingival crevicular fluid in the combination group were significantly lower than those in the routine group(all P<0.05)respectively. Conclusion Lactobacillus reuteri as adjuvant treatment of PIS can more effectively alleviate the symptoms of patients,while improves the microbial environment around the implant,and has a better therapeutic effect on PIS.
