复合保温在小儿尿道下裂手术中的应用研究 被引量:3

摘要 目的观察复合保温和常规保温在小儿尿道下裂手术中对患儿体温的影响。方法将2017年10月至2018年12月该院收治的80例择期行尿道下裂手术患儿采用随机数字表法分为复合保温组(A组)和常规保温组(B组),每组40例。监测并记录患儿麻醉前(T1)、手术开始后30min(T2)、手术结束时(T3)和出手术室时(T4)的体温,并分别测定各时间点血浆C反应蛋白和白细胞水平。记录复苏期麻醉苏醒时间、拔管时间及寒战发生情况。结果A组患儿T2、T3时体温均明显高于B组,T2~T4时血浆C反应蛋白和白细胞水平均明显低于B组,复苏期麻醉苏醒时间、拔管时间均明显短于B组,寒战发生率明显低于B组,差异均有统计学意义(P<0.05)。结论复合保温措施有助于维持小儿尿道下裂手术中体温的稳定,预防术中低体温的发生,减轻术中应激反应,有利于患儿快速康复。 Objective To investigate the effect of composite insulation and conventional insulation on the body temperature in pediatric hypospadias surgery.Methods Eighty pediatric patients undergoing elective hypospadias surgery From October 2017 to December 2018 in this hospital were divided into the composite insulation group(A) and conventional insulation group(B) by adopting the random number table method,40 cases in each group.The body temperatures were recorded before anesthesia(T1),at 30 min(T2) after surgery begin,operation end(T3),out of the operation room(T4).Plasma CRP and WBC count were measured at the each time point.The anesthetic recovery time,extubation time and shivering occurrence were recorded.Results The body temperature at T2 and T3 in the group A was significantly higher than that in the group B,plasma CRP and WBC levels at T2-T4 were significantly lower than those in the group B,the anesthetic recovery time during recovery period and extubation time of tracheal intubation in the group A were significantly shorter than those in the group B,the occurrence rate of shivering was significantly lower than that in the group B,and the differences were statistically significant( P <0.05).Conclusion The composite insulation measures contribute to maintain the temperature stability,prevent the occurrence of intraoperative hypothermia,reduce the intraoperative stress reaction and is conducive to the rapid recovery of the children patients.
