脊柱术后切口感染危险因素的荟萃分析 被引量:18

A meta-analysis of risk factors for incision infection after spinal surgery

摘要 目的分析脊柱术后切口感染的危险因素,为减少感染率提供预防措施与理论依据。方法计算机检索CNKI、万方、维普、中国生物医学文献、PubMed、Medline数据库,日期从建库至2018年7月止,检索有关脊柱术后切口感染危险因素分析的公开发表文献。采用Review maneger 5.0软件对纳入的文献数据进行分析。结果共纳入10篇文献,中文8篇,英文2篇;回顾性队列研究8篇,前瞻性队列研究2篇。荟萃分析脊柱术后切口感染危险因素的比数比(OR)值(95%CI):年龄>60岁2.42(1.60~3.27),糖尿病病史2.07(1.45~2.77),切口类型2.13(1.80~3.54),手术时间6.16(2.11~13.42),体重指数2.14(1.64~7.26),脑脊液漏5.24(3.12~8.68),置入材料1.45(1.20~3.41),输血1.67(1.43~4.56),差异均有统计学意义;而术前预防性抗菌药物使用不当1.23(0.12~2.24),吸烟3.11(0.89~3.94),差异无统计学意义。结论年龄>60岁、糖尿病病史、切口类型、手术时间、体重指数、脑脊液漏、置入材料、输血是术后切口感染的高危因素,应采取针对性的措施进行预防。 Objective To analyze the risk factors of incision infection after spinal surgery for better preventive measures and reducing infection rate.Methods CNKI,Wanfang,VIP,Chinese Biomedical Literature,PubMed,and Medline databases were searched to retrieve relevant articles about the risk factors for incision infection after spinal surgery from the date of establishment of the database to July 2018.Review Manager 5.0 software was used to analyze the included studies.Results A total of 10 studies were included,including 8 in Chinese and 2 in English.Eight were retrospective cohort studies and 2 prospective cohort studies.Meta-analysis identified the following risk factors for incision infection after spinal surgery(OR)and its 95%CI:age>60 years,2.42(1.60-3.27),diabetes history,2.07(1.45-2.77),incision type,2.13(1.80-3.54),operative time,6.16(2.11-13.42),body mass index,2.14(1.64-7.26),cerebrospinal fluid leakage,5.24(3.12-8.68),placed material,1.45(1.20-3.41),and blood transfusion,1.67(1.43-4.56).Inappropriate use of prophylactic antibiotics before operation,1.23(0.12-2.24),and smoking,3.11(0.89-3.94)were not implied in incision infection after spinal surgery.Conclusions Age>60 years old,diabetes mellitus history,incision type,operative time,body mass index,cerebrospinal fluid leakage,placed materials,and blood transfusion are high risk factors for incision infection after spinal surgery.Specific measures should be taken to prevent incision infection.
