摘要
目的探讨不完全性颈部脊髓损伤患者应用早期手术治疗的临床效果。方法纳入2018年3月~2019年3月我院行手术治疗的93例不完全性颈部脊髓损伤患者进行研究,依据随机对照原则分为观察组(n=47)和对照组(n=46)。对照组患者行常规保守治疗,观察组患者行早期手术治疗。比较两组患者治疗效果、并发症情况以及治疗前后功能独立性评定量表(FIM)评分、美国脊髓损伤协会(ASIA)评分及脊髓独立性评定量表Ⅲ中文版(SCIM-Ⅲ)评分变化情况。结果观察组患者治疗总有效率为89.36%,高于对照组的69.57%,差异显著(P<0.05);观察组患者治疗后FIM评分、ASIA评分及SCIM-Ⅲ评分均高于对照组,差异显著(P<0.05);观察组患者并发症总发生率为8.51%,低于对照组的23.91%,差异显著(P<0.05)。结论早期手术治疗不完全性颈部脊髓损伤效果优于保守治疗,能改善患者运动功能,值得临床推荐。
Objective To investigate the clinical effect of early surgical treatment for patients with incomplete injury of cervical spinal cord. Methods A total of 93 patients with incomplete injury of cervical spinal cord who were given surgery in our hospital from March 2018 to March 2019 were enrolled in the study. According to the principle of randomization, the patients were divided into the observation group of 47 cases and the control group of 46 cases. Patients in the control group were given routine conservative treatment. Patients in the observation group were given early surgical treatment. The therapeutic outcomes, complications, and changes of Functional Independence Rating Scale(FIM)scores, American Spinal Cord Injury Association(ASIA) scores, and Spinal Cord Independence Measure Ⅲ Chinese Version(SCIM-Ⅲ)scores before and after treatment were compared between the two groups. Results The total effective rate in the observation group was 89.36%, which was higher than that of(69.57%) in the control group, and the difference was significant(P<0.05);the FIM score, ASIA score and SCIM-Ⅲ score after treatment in the observation group were higher than those in the control group, and the difference was significant(P<0.05);the total incidence rate of complications in the observation group was 8.51%, which was lower than that of 23.91% in the control group, and the difference was significant(P<0.05). Conclusion The efficacy of early surgical treatment of incomplete injury of cervical spinal cord is superior to conservative treatment and can improve the motor function of patients, which is worthy of clinical recommendation.
