皮层下动脉硬化性脑病应用1.5TMRI常规序列及MRA诊断的价值探析

摘要 目的:探究1.5T磁共振成像(MRI)常规序列及磁共振血管造影(MRA)对皮层下动脉硬化性脑病(SAE)的诊断价值。方法:回顾性分析我院106例疑似SAE患者1.5TMRI常规序列、MRA检查资料,分别与磁共振波普成像(MRS)结果比较,探究其诊断价值。结果:MRI诊断正确89例(83.96%),漏诊3例(2.83%),误诊14例(13.21%);MRA诊断正确103例(97.1 7%),漏诊1例(0.94%),误诊2例(1.89%)。MRI与MRA诊断SAE的特异度(75.00%vs25.00%)、阳性预测值(99.01%vs96.70%)比较,差异无统计学意义(P>0.05);但MRA诊断SAE的灵敏度(98.04%vs86.27%)、阴性预测值(60.00%vs6.67%)及准确率(97.17%vs83.96%)均高于MRI(P<0.05)。结论:1.5TMRI常规序列对SAE的诊断效果不及MRA检查,但2者联合诊断可综合评估患者脑血管及脑实质变化情况情况,对后续临床治疗具有指导意义。 Objective: To explore the diagnostic value of 1.5 T magnetic resonance imaging(MRI) routine sequence and magnetic resonance angiography(MRA) in subcortical arteriosclerotic encephalopathy(SAE). Methods: The data of 1.5 TMRI routine sequence and MRA examination of 106 suspected SAE patients in our hospital were retrospectively analyzed, and the data were compared with results of magnetic resonance spectroscopy(MRS). Results:There were 89 cases(83.96 %) of correct diagnosis, 3 cases(2.83 %) of missed diagnosis and 14 cases(13.21 %) of misdiagnosis by MRI diagnosis. There were 103 cases(97.17 %) of correct diagnosis, 1 case(0.94 %) of missed diagnosis and 2 cases(1.89 %) of misdiagnosis by MRA diagnosis. There were no significant differences in the specificity(75.00 % vs 25.00 %) and positive predictive value(99.01 % vs 96.70 %) between MRI and MRA in the diagnosis of SAE(χ~2=0.500、0.157,P=1.249、0.264). However, the sensitivity, negative predictive value and accuracy rate of MRA for diagnosis of SAE were higher than those of MRI [(98.04 % vs 86.27 %),(60.00 % vs 6.67 %),(97.17 % vs 83.96 %)](χ~2=9.767、4.077、10.821,P=0.002、0.008、0.001). Conclusion:1.5 TMRI routine sequence is inferior to MRA in the diagnosis of SAE, but the combined diagnosis of the two can comprehensively evaluate the changes of cerebral vessels and brain parenchyma, and it is of guiding significance for subsequent clinical treatment.
