摘要
目的探讨原发性免疫性血小板减少性紫癜(ITP)患者使用重组人血小板生成素(rh TPO)治疗前后外周血IL-2、IL-4、CD19^+细胞、B1细胞水平及其临床意义。方法采用CBA技术检测65例ITP患者治疗前后及38例对照组外周血IL-2、IL-4、CD19^+细胞及B1细胞水平变化。结果治疗前ITP组外周血IL-2水平及IL-2/IL-4比值明显高于对照组,而IL-4水平明显低于对照组,CD19^+细胞及B1细胞水平明显高于对照组,差异均有统计学意义(P <0.05);经rh TPO治疗后IL-2水平及IL-2/IL-4比值较治疗前明显下降,CD19^+细胞及B1细胞水平也有所下降,IL-4水平较治疗前明显上升,差异均有统计学意义(P <0.05)。结论 ITP患者存在着T淋巴细胞和B淋巴细胞的异常表达,ITP是一种以Th1优势伴B1细胞高表达的免疫性疾病,TPO治疗后血小板上升,可能通过调节ITP患者外周血T淋巴细胞及B淋巴细胞水平而发挥作用。
Objective To study the expression and clinical significance of the levels of IL-2,IL-4,CD19^+ cell,B1 cell of idiopathic thrombocytopenic purpura ( ITP) patients before and after recombinant human thrombopoietin ( rhTPO) therapy. Methods The changes of serum IL-2,IL-4,CD19^+ cell,B1 cell levels were measured by CBA in 65 patients with ITP before and after rhTPO therapy and 38 persons for heath examination. Results The levels of IL-2 and IL-2 /IL-4 in peripheral blood of 65 patients in the ITP group were significantly higher than those in the control group,while the IL-4 level was significantly lower than that in the control group. The levels of CD19^+ cells and B1 cells were significantly higher than those in the control group,with the differences statistically significant ( P < 0.05). After treatment with rhTPO,IL-2 level and IL-2 /IL-4 ratio decreased significantly,CD19^+ cells and B1 cells also decreased,IL-4 levels increased significantly,and the differences were statistically significant compared with that before treatment ( P < 0.05). Conclusion Peripheral blood T lymphocyte and B lymphocyte were expressed abnormally in ITP patients,and ITP is a Th1 lymphocyte dominated disease. The level of platelets rose after rhTPO treatment,rhTPO may play a role in regulating the migration of T lymphocyte and B lymphocyte in peripheral blood of ITP patients.
