生理盐水双段注射方法在头颈CTA成像检查中的应用研究 被引量:3

摘要 目的探讨生理盐水双段注射方法对头颈CTA检查中成像效果的影响。方法选取2018年1月至2019年3月首都医科大学附属北京友谊医院收治的60例行头颈血管CT造影检查的患者为前瞻性研究对象,将患者随机分为两组,每组各30例。对照组患者采后生理盐水追加法:先行注射碘普罗胺370,注射总量为50 ml,注射流率为4 ml/s。随后以相同的注射流率追加注射生理盐水40 ml。实验组患者采后生理盐水双段注射法,先行注射生理盐水20 ml,注射流率为4 ml/s,随后以相同的注射流率注射碘普罗胺370,注射总量为50 ml,并追加注射生理盐水40 ml。采用对比剂追踪触发技术,触发阈值设定为100 Hu,扫描结束后记录、测量并绘制监测层面不同兴趣区的时间密度曲线。分别测量两组患者的主动脉、左右颈总动脉和大脑中动脉的CT值、客观噪声(SD),并计算信噪比(SNR)。由两名放射科医师对两组患者进行主观评分。结果两组图像主动脉、双侧颈总动脉及大脑中动脉的CT值、SD值及SNR差异均无统计学意义(P> 0. 05);对比剂触发前监测时间段的时间密度曲线比较,实验组在主动脉监测层CT值均值上升段较对照组平缓,差异具有统计学意义(P <0. 05);两组图像目标血管主观评分差异无统计学意义(P> 0. 05);实验组上腔静脉硬化伪影评分优于对照组,差异具有统计学意义(P <0. 05)。结论应用生理盐水双段注射方法行头颈CTA检查,可改善触发监测区CT值变化的稳定性,并提高对比剂最佳扫描时相的准确性,能够获得满意的图像质量。 Objective To investigate the effect of double period saline injection on the imaging of head and neck CTA. Methods 60 patients with head and neck CT angiography admitted to Beijing Friendship Hospital of Capital Medical University from January 2018 to March 2019 were enrolled in this study. The patients were randomly divided into two groups, with 30 cases in each group. The normal saline addition method was used in the control group, iodoprolamine 370 was injected first, with a total injection volume of 50 ml, and the injection flow rate was 4 ml/s, then 40 ml normal saline was additionally injected with the same flow rate. In the experimental group, 20 ml normal saline was injected with a flow rate of 4 ml/s, followed by iodoprolamine 370 at the same flow rate with a total injection of 50 ml, and 40 ml normal saline was added. Contrast agent tracking trigger technology was adopted, and the trigger threshold was set to 100 Hu. After scanning, the time density curves of different interest zones at the monitoring level were recorded, measured and drawn. CT value and objective noise (SD) of aorta, left and right common carotid artery and middle cerebral artery of the two groups were measured, and SNR was calculated. Subjective scores were performed by two radiologists. Results There were no significant differences in CT, SD and SNR between the two groups of images of the aorta, bilateral common carotid artery and middle cerebral artery ( P >0.05). Compared with the time density curve of the monitoring period before the trigger of the agent, the mean CT value of the monitoring layer of the aorta in the experimental group was gentle compared with that in the control group, and the difference was statistically significant ( P <0.05). And there was no significant difference in subjective scores between the two groups ( P >0.05). The false evaluation score of superior vena cava sclerosis in the experimental group was better than that in the control group, and the difference was statistically significant ( P <0.05). Concl
