摘要 目的心脏损伤是癌症患者术后化疗的常见副作用,及时发现化疗患者心肌损害可有助于提高患者生活质量。该研究主要评价二维斑点追踪自动功能成像技术(AFI)在化疗后心肌损害中的临床应用价值。方法选取该院2018年1月—2019年1月收治的42 例患恶性肿瘤且完成手术,预期生存超过12个月的化疗患者,与首次化疗后半年行AFI 检查和常规胸超检查心脏功能,对比两种检测的手段对患者心肌损坏的敏感性,并利用统计学方法分析不同方法对心功异常的诊出效果。结果 42 例化疗患者中,其中35例患者AFI 牛眼图提示左室缺血,占比83.33%,而普通胸超仅提示10例患者发生LVEF减低,检出率为23.81%,AFI对化疗患者心肌损害的敏感性显著高于胸超,两者检查结果对比差异有统计学意义(χ^2=6.481,P<0.01)。结论 AFI对化疗患者的心肌损害检出率显著高于胸超,从而帮助临床医生对患者进行早期干预,提高患者生活质量。 Objective Cardiac injury is a common side effect of postoperative chemotherapy in cancer patients. It is helpful to find that myocardial damage in chemotherapy patients can improve the quality of life of patients. This study mainly evaluates the clinical application value of two-dimensional speckle tracking automatic functional imaging (AFI) in myocardial damage after chemotherapy. Methods A total of 42 patients with malignant tumors who underwent surgery from January 2018 to January 2019 in our hospital were enrolled. Patients who were expected to survive for more than 12 months were treated with AFI and routine chest ultrasound for cardiac function after the first chemotherapy. To compare the sensitivity of the two methods of detection to myocardial damage in patients, and to use statistical methods to analyze the effect of different methods on the diagnosis of cardiac function abnormalities. Results Of the 42 patients with chemotherapy, 35 of them had AFI bull's eye plot suggesting left ventricular ischemia, accounting for 83.33%, while the common chest ultrasound only suggested that 10 patients had LVEF reduction, the detection rate was 23.81%. AFI for chemotherapy sensitivity of myocardial damage was significantly higher than that of the chest, and the results of the two tests were statistically significant (χ^2= 6.481, P<0.01). Conclusion AFI has a significantly higher detection rate of myocardial damage in patients with chemotherapy than chest ultrasound, which helps clinicians to intervene early in patients and improve their quality of life.
