摘要
目的研究急性大面积缺血性卒中患者脑电图(EEG)分级与神经功能预后的相关性。方法选取我院2016年8月至2018年3月收治的60例急性大面积缺血性卒中患者为研究对象,随访90 d对其进行改良Rankin量表(m RS)评分,其中≤2分者30例作为转归良好组,>2分者30例作为转归不良组。比较两组患者的EEG分级、NIHSS、ASTRAL、m RS评分,同时采用Pearson相关性分析法分析EEG分级与NIHSS、ASTRAL、m RS评分的相关性,计算EEG分级预测预后的灵敏度、特异度、阳性预测值、阴性预测值和准确度。结果转归不良组的EEG分级级数、NIHSS、ASTRAL、m RS评分均明显高于转归良好组(P<0.05);Pearson相关性分析结果显示,EEG分级与NIHSS、ASTRAL、mRS评分具有显著相关性(P<0.05);EEG分级预测预后的灵敏度、特异度、阳性预测值、阴性预测值和准确度均较高。结论急性大面积缺血性卒中患者EEG分级与神经功能预后具有显著相关性,EEG分级越高,神经功能预后越差,对临床诊断和治疗具有重要意义,值得推广。
Objective To study the correlation between electroencephalogram (EEG) grading and prognosis of neurological function in patients with acute massive ischemic stroke. Methods A total of 60 patients with acute massive ischemic stroke admitted in our hospital from August 2016 to March 2018 were selected as the study objects. The patients were followed up for 90 days and scored by modified Rankin Scale (mRS). Among them, 30 patients with mRS<2 scores were regarded as good prognosis group and 30 patients with mRS>2 scores were regarded as poor prognosis group. The EEG grading and scores of NIHSS, ASTRAL and mRS were compared between the two groups. Pearson correlation analysis was used to analyze the correlation between EEG grading and scores of NIHSS, ASTRAL and mRS. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of EEG grading in predicting prognosis were calculated. Results The EEG grading and scores of NIHSS, ASTRAL and mRS of the poor prognosis group were significantly higher than those of the good prognosis group (P<0.05). Pearson correlation analysis showed that EEG grading were significantly correlated with scores of NIHSS, ASTRAL and mRS (P<0.05). The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of EEG grading in predicting prognosis were higher. Conclusion The EEG grading of patients with acute massive ischemic stroke is significantly correlated with neurological prognosis. The higher the grade of EEG, the worse the neurological prognosis. It is of great significance for clinical diagnosis and treatment and is worth popularizing.
