克氏针操纵复位在弹性髓内钉固定治疗儿童股骨干骨折的临床效果 被引量:5

摘要 目的探讨克氏针操纵复位在弹性髓内钉固定治疗儿童股骨干骨折的临床效果。方法回顾性分析2017年1月~2018年10月我院收治的46例儿童股骨干骨折患儿的临床资料,按照治疗方法的不同将其分为K组(5例)与Y组(41例),K组采用克氏针操纵复位弹性髓内钉固定,Y组采用有限切开复位弹性髓内钉固定。比较两组的术中出血量、手术时间、X线曝光次数、骨折愈合时间;比较两组治疗后的临床疗效、膝关节功能评分(HSS)及并发症总发生率。结果K组术中出血量少于Y组,手术时间、骨折愈合时间短于Y组,X线曝光次数多于Y组,HSS评分低于Y组,差异均有统计学意义(P<0.05);两组患儿的优良率比较,差异无统计学差异(P>0.05);两组的术后并发症总发生率比较,差异无统计学差异(P>0.05)。结论儿童股骨干骨折采用弹性髓内钉治疗,克氏针操纵复位治疗操作方便,耗时少,创伤小,能有效降低术后疼痛的发生率,加快术后骨折愈合进程,改善膝关节功能,但X线曝光次数较多,值得临床推广。 Objective To investigate the clinical effect of Kirschner wire manipulation and reduction in the treatment of femoral shaft fracture in children with elastic intramedullary nailing. Methods The clinical data of 46 children with femoral shaft fracture admitted to our hospital from January 2017 to October 2018 were retrospectively analyzed. According to different treatment methods, they were divided into K group (5 cases) and Y group (41 cases). Children in group K were treated with Kirschner wire for reduction and elastic intramedullary nailing, and children in group Y were treated with limited open reduction and elastic intramedullary nailing. The intraoperative blood loss, operation time, X-ray exposure, and fracture healing time were compared between the two groups. The knee function score (HSS) and total incidence rate of complications were compared between the two groups. Results The HSS score of the K group was lower than that of the Y group, the operative time was shorter than that of the Y group, the fracture healing time was shorter than that of the Y group, and the X ray exposure frequency was more than that of the Y group, all the differences were statistically significant (P<0.05). There was no significant difference in the excellent and good rates between the two groups (P>0.05). There was no significant difference in the total incidence of postoperative complications between the two groups (P>0.05). Conclusion Children femoral shaft fracture are treated with elastic intramedullary nail, which is convenient, time-consuming and minimally invasive, and can effectively reduce the incidence of postoperative pain, accelerate the postoperative fracture healing process, and improve knee joint function. However, X-ray exposure is more frequently, which is worthy of clinical promotion.
