摘要
目的:研究二孩生育意愿对初产妇分娩方式选择及最终分娩方式,尤其是孕妇要求剖宫产的影响。方法:纳入2018年2月至2018年8月在重庆医科大学附属第一医院行规律产前检查的孕中期单胎初产妇,按是否有生育二孩意愿分为有生育二孩意愿组、无生育二孩意愿组,比较两组孕中期、孕晚期意愿分娩方式及最终的分娩方式及类型。对两组基本人口学变量进行单因素分析及多因素Logistic回归分析。结果:632例完成所有调查并分娩的孕妇纳入最终分析,其中有生育二孩意愿组孕妇321例,无生育二孩意愿组孕妇311例。两组在孕中期、孕晚期以及最终分娩方式的比较,有生育二孩意愿组的孕妇要求剖宫产的比例显著低于无生育二孩意愿组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析表明,有生育二孩意愿和年龄(20~29岁)在孕中期、孕晚期以及最终分娩方式的选择上均是孕妇要求剖宫产的保护因素(校正OR<1,P<0.05)。结论:随着二孩政策的全面放开,有生育二孩意愿及30岁以下的初产妇,会更少考虑剖宫产,这一变化可以从孕中期持续到分娩结束。
Objective:To assess the influence of second-child intent on delivery mode preferences and final delivery modes in nulliparous women,particularly caesarean delivery on maternal request(CDMR).Methods:A pilot cohort study was conducted in The First Affiliated Hospital of Chongqing Medical University including nulliparous women in second trimester with singleton pregnancy and regular prenatal visit.They were divided into two groups based on whether they intended to have a second child.Their preferred mode of delivery was compared in second trimester,at admission and labor.Univariate analysis and multivariate Logistic regression were performed on the basic characteristics in the two groups.Results:632 women who completed all interviews and delivered were enrolled in final analysis.321 women intended to have a second child,whereas 311 women did not.The proportion of CDMR in intend group was significantly lower than that of the other group in second trimester,third trimester and at labor(P<0.05).Multivariate Logistic regression showed that intention to have a second child and younger age(20-29 years old) were both protective factors in the three stages(adjusted OR<1,P< 0.05).Conclusions:With the full liberalization of the second child policy,nulliparous women whowere younger then 30 years old and intended to have a second childwere less likely to request CD.This difference could last from second trimester until labor.
