摘要
目的对三亚本土青年缺血性脑卒中的危险因素、病因分型TOAST(Trial of Org 10172in Acute Stroke Treatment)分型进行初步探讨及分析.方法回顾分析2012年6月-2018年6月于解放军总医院原海南分院神经内科和三亚市人民医院神经内科住院的90例本土青年缺血性脑卒中(18~45岁)患者青年组,随机抽取同期于解放军总医院海南分院神经内科住院的本土中老年缺血性脑卒中患者98例中老年组,对2组患者的危险因素及病因分型进行分析.结果青年组男71例(78.9%),女19例(21.1%),与中老年组性别构成比较有明显差异(P=0.039).青年组主要危险因素依次为吸烟(40%)、高血压病(40%)、饮酒(35.6%)和脂代谢异常(14.4%).青年组TOAST分型分布依次为小动脉闭塞型(51.1%)、大动脉粥样硬化型(42.2%)、心源性栓塞型(3.3%)、其他原因型(3.3%)、无不明原因型,与中老年组比较无显著差异(P>0.05).结论三亚本土青年缺血性脑卒中患者在人口学特征、危险因素及病因学分型方面有其独有的特征.
Objective To discuss and analyze the risk factors and etiology classification of Sanyayoung adults with ischemic stroke.Methods From June 2012 to June 2018,a total of 90 consecutive local young ischemic stroke patients(18 to 45 years old)were admitted to the department of neurology in Hainan branch of PLA general hospital and Sanyapeople's hospital.These patients were enrolled in young group,while 98 middle- aged and old local patients with ischemic stroke were enrolled in middle-aged and old group.The risk factors of stroke and the distribution of stroke subtype were studied retrospectively.Results The young group contained 71men(78.9%)and 19 women(21.1%).Compared with middle-aged and old group,there was significant difference of sex constitution.The 4 most common risk factors were smoking(40%),hypertension (40%),alcoholism (35.6%).hyperlipidemia(14.4%).Regarding stroke subtype,TOAST classification of young ischemic stroke was as follows:small-vessel occlusion (SAO)51.1%,large-artery atherosclerosis (LAA)42.2%,cardioembolism (CE)3.3%,other determined etiology(OC)3.3%,and no undetermined etiology( UND),which was similar to the middle-aged and old group.Conclusion The demographic characteristic, risk factors and distribution of stroke subtypes in local young adults with ischemic stroke were different. We should pay more attention to local young adults with ischemic stroke in Sanya.
