摘要
目的研究上海市浦东新区计算机断层扫描(computed tomography,CT)、磁共振成像(magnetic resonance imaging,MRI)和数字减影血管造影(digital subtraction angiography,DSA)设备利用的适宜性及其影响因素.方法在该区9所公立医院中,随机抽取和检查了1425份接受过CT、MRI和DSA检查的住院病案.专家根据检查表,对设备利用的适宜性进行评价.结果在CT、MRI、DSA检查中,87.08%、87.68%和96.17%的检查是适宜的;分层logistic回归显示,医疗机构、疾病类型、检查次数是CT和MRI检查必要性的影响因素.结论上海市浦东新区三类设备利用适宜性整体较高,但应关注不同疾病类型和不同医院间的CT和MRI设备利用合理性的差异.
Objective To analyze the appropriateness in utilization of computed tomography(CT),magnetic resonance imaging(MRI) and digital subtraction angiography(DSA) in Pudong New Area of Shanghai and the relevant influencing factors. Methods A total of one thousand four hundred and twenty-five medical records of the inpatients in nine public hospitals of Pudong New Area were randomly sampled and audited. Experts evaluated the appropriateness in utilization of CT,MRI and DSA by using check lists. Results Among the examinations of CT,MRI and DSA,87.08%,87.68% and 96.17% were used appropriately. Multi-level logistic model showed that medical institutions,disease type and examination times were the factors influencing the necessity of CT and MRI utilization. Conclusion Three types of large equipments in Pudong New Area of Shanghai has been used appropriately as a whole. However,it is suggested to pay attention to the differences in utilizating CT and MRI among different types of diseases in different hospitals.
